摘要 目的 研究急性重症胰腺炎 (SAP)中细胞凋亡现象及bcl 2与rnkp 1基因对SAP中细胞凋亡的影响。方法 将 3 6只Wistar大鼠随机分为 6组 ,即正常对照组、SAP模型制作后 1、2、4、8、16h组。利用原位末端标记法和琼脂糖凝胶电泳检测凋亡小体及凋亡梯形。采用定量逆转录聚合酶链式反应检测凋亡调控基因bcl 2、rnkp 1基因mRNA转录水平。 结果 模型制作后 2h组即可检出凋亡小体及梯形 ,4h组凋亡小体明显增多至高峰并出现规整的凋亡梯形。胰腺组织bcl 2基因mRNA表达水平 2h组达高峰 0 .87± 0 .11,并于 8、16h组降至正常。各组中脾细胞rnkp 1基因mRNA表达水平于 8h组可见增高 0 .5 4± 0 .17,16h组达高峰 0 .63± 0 .0 9。结论 大鼠SAP的发病过程中 ,同时存在细胞坏死和凋亡。细胞凋亡现象可能具有保护与加重损害的双重作用。rnkp 1和胰蛋白酶原编码基因活性区的同源性可能是急性重症胰腺炎晚期病情不可逆转的因素之一。 Objective To investigate the apoptosis in severe acute pancreatitis (SAP) and the potential effects of rnkp 1 and bcl 2 genes on the apoptotis of SAP.Methods Thirty six SAP rats were randomly divided into 6 groups (n=6 each group): normal control groups and the groups of 1,2,4,8 and 16 h after model development.Apoptosis body and ladder were individually detected by using in situ end labeling method (ISEL) and agrose gel electrophoresis.The transcription levels of rnkp 1 mRNA and bcl 2 mRNA were assayed by using quantitative reverse transcription polymerase chain reaction.All experimental rats were sacrificed for pathological examination of pancreas.Results In the postoperative 2?h group,apoptosis ladder and apoptosis body were detected.Apoptosis body tended to increase and presented highest on the 4th h.In the postoperative 4?h group,considerably typical apoptosis ladder was detected.mRNA express level of bcl 2 in pancreas tissue appeared higher on the first h and highest on the second h (0.87±0.11),then returned to the normal level on the 8th h and 16th h.No alteration of mRNA express level of rnkp 1 in spleen tissue occurred in the postoperative 1?h,2?h and 4?h groups.This express level tended to increase and presented highest on the 16th h (0.63±0.09).Conclusion Two different death patterns,named cell necrosis and apoptosis,present in the pathological course of experimental rat SAP.In rat SAP,apoptosis might play dual parts to protect and/or damage pancreas tissue.The active region homogeneity between rnkp 1 and trypsin coding genes might be one of the irreversible reasons in late SAP.
