摘要
目的 :探讨膝关节部骨巨细胞瘤治疗的术式选择。方法 :6 0例膝关节部骨巨细胞瘤病例根据恶性程度分别采取刮除植骨 ,大块切除、髌骨重建 ,人工膝关节置换 ,膝关节融合 ,截肢等不同手术方式进行治疗。通过随访观察疗效。结果 :术后复发 4例。骨移植病例膝关节功能按Mankin标准评定 ,优 2 9例 ,良 6例 ,可 4例 ,差 3例。人工膝关节置换病例未出现感染、松动和折断现象。结论 :术前对骨巨细胞瘤恶性程度进行判定 ,采用相应术式加以治疗可获得较好的治疗效果。
Objective: Our aim was to discuss the proper method of operation in treating giant cell tumor of bone(GCT)in knee. Methods: According to the malignant degree, different operations, such as curettage followed by autogenous bone graft, wide resection and reconstruction with patella, artificial knee joint replacement, arthrodesis and amputation, were performed to treat 60 patients with GCT in knee. The patients was followed to observe the therapeutic effect. Results: Four cases recurred after operation. In the patients received autogenous bone graft, 29 were evaluated as excellent for the therapeutic effect, 6 as good, 4 as fair, and 3 as bad according to Mankin criteria. No infection, loosening and fracture of implant occurred in patients received joint replacement. Conclusion: Favorable prognosis can be achieved by choosing proper operation method according to the malignant degree of GCT.
