摘要
目的 探讨肝癌患者脾脏T细胞亚群免疫状态。方法 采用流式细胞技术 (FCM )检测 3 1例肝癌患者外周及脾静脉血T细胞亚群CD 4+ ,CD 8+ ,CD4+ /CD8+ (以下简称CD 4,CD 8,CD 4/CD 8)的变化 ,并以 13例肝硬化患者作为对照组。结果 6例Ⅰ期肝癌患者脾静脉血CD 4高于肝硬化组 ,外周血CD 8低于肝硬化组 (P <0 .0 5 ) ,外周及脾静脉血CD 4/CD8高于肝硬化组 (P <0 .0 1)。 12例Ⅱ期、13例Ⅲ期肝癌患者外周及脾静脉血CD4,CD 4/CD8低于肝硬化组 ,CD 8高于肝硬化组 (P <0 .0 1或 P <0 .0 5 ) ,且脾静脉血CD4,CD 4/CD 8低于外周血 ,CD 8高于外周血 (P <0 .0 5 )。结论 早期肝癌患者 ,T细胞亚群处于良好抗肿瘤免疫状态 ,脾脏在维持机体T细胞亚群平衡中起重要作用。随肿瘤进展 ,机体免疫功能逐渐受损 ;中晚期肝癌患者T细胞亚群紊乱 ,机体处于免疫抑制状态 ,尤其以脾脏T细胞亚群紊乱更为明显 ,出现了负性抗肿瘤免疫作用 ,加重了机体免疫抑制。
Objective To study T lymphocyte subsets immunological condition of spleen in primary hepatocellular carcinoma (PHC) with liver cirrhosis. Methods T lymphocyte subsets such as CD4,CD8,CD4/CD8 in peripheral and spleen venous blood in 31 cases of PHC with liver cirrhosis were detected by FCM.and 13 patients with liver cirrhosis were observed as the control group. Results In 6 patients with stage Ⅰ liver cancer, CD8 of peripheral venous blood significantly decreased (P<0.05);CD4 of spleen venous blood, CD4/CD8 of peripheral and spleen venous blood much higher than those of control group (P<0.01).In 12 patients with stage Ⅱ PHC and 13 patients with stage Ⅲ PHC,CD4,CD4/CD8 in peripheral and spleen venous blood significantly decreased and CD8 significantly increased compared with those in liver cirrhosis group (P<0.01 or P<0.05);but CD4, CD4/CD8 in spleen blood were significantly lower and CD8 was obviously higher than those in peripheral venous blood.Conclusions T lymphocyte subsets have well antitumor immun function in early PHC, the spleen plays an important role in keeping T lymphocyte subsets balance.As tumor advanced, the host immune function was gradually impaired, T lymphocyte subsets became imbalance in advanced PHC, the host immune function was inhibited, espicially, the T lymphocyte subsets of spleen became obviously imbalance,so the spleen can produce antitumor immunosuppression and weaken the host immune response.
