摘要
目的探讨基层医院开展经皮肾镜取石术(PCNL)疗效和经验。方法 78例上尿路结石患者采用PCNL治疗,分析手术疗效,总结手术经验。结果所有患者手术均穿刺成功,2例中转开放手术,76例患者均行一期碎石术,其中74例行单通道取石,2例双通道取石,平均手术时间为56.38 min,结石清除率96.15%,3例术后继发出血,1例保守治疗治愈,2例术后出血行超选择性肾动脉栓塞术治疗,无丢肾、无败血症、无尿瘘、气胸、肠管损伤等严重并发症。结论基层医院根据自身情况开展PCNL安全可靠,疗效确切,但要严格把握适应证、充分术前准备、循序渐进。
Objective To evaluate the efficacy and sum up the experience of the percutaneous nephrolithotomy(PCNL)in a basic hospital.Methods From June 2012 to May 2014,78 patients underwent PCNL in treatment of upper urinary calculi in our hospital,and the clinical data were retrospectively collected.Then the efficacy and experience of this operation were retrospectively analysed.Results All op-erations were punctured successfully,and 2 cases were transferred to open surgery.All 76 patients underwent lithoclasty in one stage,and 74 of them were removed caculas from single-channel,2 of them were removed caculas from double-channel.The average operating time was 56.38 min.The stone-free rate was 96.15%.Three cases had secondary bleeding after operation,and 1 of them was cured by conservative management,2 of them were treated by super-selective renal artery embolization,without serious complications like kidney loss,sepsis,urinary fistula,pneumothorax,and bowel injury.Conclusion PCNL in the basic hospital is safe,reliable and effective,but it is also very important to strictly control the indications of the operation and follow in proper sequence,and adequate pre-operation preparation as well.
