肌电生物反馈治疗脑卒中偏瘫患者肩关节运动功能的疗效研究 被引量:10

Effect of electromyographic biofeedback treatment in improving shoulder joint motion functioning of patients with hemiplegia stroke

摘要 目的探讨肌电生物反馈(EMG-BF)治疗脑卒中偏瘫患者肩关节运动功能的疗效。方法 40例脑卒中偏瘫患者随机分为研究组(20例)和对照组(20例),两组患者均予以常规康复治疗,研究组在常规康复治疗的基础上予以EMG-BF治疗。治疗前及治疗6周后评估两组患者偏瘫侧肩关节外展主动活动度(AROM)、肩关节主动外展肌电均值(AEMG),Fugl-Meyer运动功能量表(FMA)评估上肢运动功能,功能独立性测量量表(FIM)评估患者日常生活自理能力。结果治疗后两组患者偏瘫侧肩关节外展的AROM与AEMG、FMA评分、FIM评分较治疗前改善(P<0.05),且研究组优于对照组(P<0.05);肩关节外展AEMG与AROM呈正相关(P<0.05)。结论 EMG-BF治疗可改善脑卒中偏瘫患者患肩运动功能,脑卒中偏瘫患者肩关节外展AEMG评分与AEMG评分呈密切相关。 Objective To discuss promoting upper limb function in patients with hemiplegia following stroke rehabilitation methods. Methods A total of 40 patients were enrolled and were randomly assigned to two groups: a group treated with EMG-BF treatment( study group,20 patients) and an untreated group( control group,20 patients). Both groups participated in a hemiplegia rehabilitation program consisting of normal rehabilitation therapy. Shoulder joint outreach initiative of motion in hemiplegia side( AROM),average electrical values of shoulder joint active abductor( AEMG),upper part movement function by Fugl-Meyer movement function score( FMA),and self-care activities by functional independence measure( FIM) were evaluated in two groups of patients before and 6 weeks after treatment. Results We found statistically significant improvement posttreatment in the AROM,AEMG,FMA,FIM( P < 0. 05),which were better than those of the control group( P < 0. 05). Stroke patients’ shoulder joint outreach AEMG and AROM were positively correlated. Conclusion EMG-BF treatment can improve the stroke shoulder movement function. Stroke patients’ shoulder joint outreach AEMG and AROM are closely related.
