Mini患者来源异种移植模型指导化疗可延长胆囊癌患者的生存期

摘要 背景与目的胆囊癌具有高度侵袭性和化疗抵抗性,目前缺少一致的治疗策略来指导一线化疗。然而,患者来源异种移植(patient-derived xenograft,PDX)模型作为一种有效模型,已越来越多地用于化疗敏感性的临床前研究。方法采用来自12例胆囊患者的新切除的原发病灶建立mini-PDX模型,检测5种最常用化疗药物的敏感性,包括吉西他滨、奥沙利铂、5-氟尿嘧啶、纳米颗粒白蛋白结合(nanoparticle albumin-bound,nab)–紫杉醇和伊立替康。基于此检测结果指导术后辅助化疗药物的选择。采用KaplanMeier方法比较45例接受吉西他滨和奥沙利铂常规化疗患者的总生存(overallsur vival,OS)和无病生存(disease free survival,DFS)。结果 Mini-PDX模型的细胞活力检测结果显示出药物反应的显著异质性。Kaplan-Meier分析结果表明,PDX指导化疗组患者的中位OS(18.6个月;95%CI:15.9–21.3个月)显著高于常规化疗组(13.9个月;95%CI:11.7–16.2个月)(P=0.030;HR=3.18;95%CI:1.47–6.91)。PDX指导化疗组患者的中位DFS(17.6个月;95%CI:14.5–20.6个月)也显著高于常规化疗组(12.0个月;95%CI:9.7–14.4个月)(P=0.014;HR=3.37;95%CI:1.67–6.79)。结论使用mini-PDX模型指导化疗方案的选择可以改善胆囊癌患者的预后。
