摘要
目的:探讨腹腔镜胆囊切除术中胆囊管变异的有效处理手段。方法:选取我院行腹腔镜胆囊切除术中出现胆囊管变异的40例患者为研究对象,术后给予胆囊壶腹分离、保持胆囊三角处空虚等处理。结果:40例胆囊管变异患者经相应处理后均顺利完成腹腔镜胆囊切除术,未出现中转开腹及胆道损伤情况。手术平均时间(50.2±10.3)min,术中平均出血量(30.8±12.9)ml,均治愈出院且随访半年未出现并发症。结论:术中准确分离胆囊壶腹、保持三角区空虚,可有效认识胆囊管变异,防范胆管损伤。
Objective： To investigate the effective way to handle the variation of cystic duct during laparoscopic cholecystectomy. Methods： 40 patients appeared the variation of cystic duct during laparoscopic cholecystectomy were selected as the subjects. After sugeries, the gallbladder ampulla of the patients was separated and the gallbladder triangle was kept in emptiness. Results： The lapa roscopic cholecystectomy of 40 patients with variation of cystic duct had been managed successfully after proper treatment, no conversion to laparotomy and bile duct injury occurred during operations. The mean operative time was（50.2±10.3）rain, and the mean bleeding volume was（30.8±12.9）ml. All patients have been cured and no complications occurs after 6 months＇ follow-up. Conclusion.The accurate separation of the gallbladder ampulla and the emptiness keeping of the gall- bladder triangle may effectively know the variation of the cystic duct and prevent the injury of the cystic duct.
