两种镇痛方式在骨科手术后疼痛管理中的应用研究 被引量:12

摘要 目的观察两种镇痛方式在骨科手术后疼痛管理的效果。方法按镇痛方式不同将803例骨科术后患者分为两组,使用无线镇痛泵系统镇痛的400例患者为A组,使用机械镇痛泵镇痛的403例患者为B组。观察患者术后2h、6h、12h、24h、48h镇痛效果,术后48h内疼痛处理时间,术后48h内患者按压自控镇痛键的总次数及有效次数;测评患者对镇痛治疗的满意度。结果两组患者术后24h、48h疼痛强度比较差异无统计学意义(P>0.05);术后2h、6h、12h时A组患者疼痛强度低于B组,组间比较差异有统计学意义(P<0.01)。疼痛处理平均时间A组(11±4.3)min、B组(17±4.9)min,两组比较差异有统计学意义(P<0.01)。患者按压自控镇痛键的总次数,A组(5.5±8)次、B组(4.8±9)次,两组比较差异无统计学意义(P>0.05);患者按压自控镇痛键的有效次数,A组(3.2±4)次、B组(2.8±4)次,两组比较差异无统计学意义(P>0.05)。患者对镇痛总体满意度A组为98.75%、B组为84.62%,两组比较差异有统计学意义(P<0.01)。结论无线镇痛泵系统有利于提高手术后疼痛管理质量。 Objective To observe the effect of two analgesia methods in pain management for patient after orthopaedic surgery. Method Divide 803 patients after orthopaedic surgery into two groups according to two analgesia methods. 400 patients use wireless analgesic pump system are in group A. 403 patients use traditional mechanical pump are in group B. Observe the analgesia effect 2 h, 6 h, 12 h, 24 h, and 48 h after surgery. Observe pain handling time in 48 h after surgery. Observe the.total times and effective times of pressing self-control button in 48h after surgery. Assess the total satisfaction level on analgesia therapy. Result There is no significant difference on pain level 24 h and 48 h after surgery between two groups (P〉0.05). Pain level 2 h, 6 h, and 12 h after surgery is lower in group A than in group B, the difference is significant (P〈0. 01). The average handling time of pain is (11±4. 3) rain in group A and (17±4. 9) min in group B, the difference is significant (P〈0. 01). The total times of pressing self--control button is (5. 5±8) in group A and (4. 8±9) in group B, there is no significant difference (P〉0. 05). Effective times of pressing self--control button is (3. 2±4) in group A and (2. 8±4) in group B, there is no significant difference (P〉0.05). Satisfaction level is 98. 75% in group A and 84. 62% in group B, the difference is significant (P〈0. 01). Conclusion Wireless analgesic pump system can help pain management after surgery.
