摘要
目的观察肠内营养支持对脑卒中患者体内血浆胰高血糖素样肽-1(GLP-1)水平及细胞免疫的影响。方法 85例脑卒中患者,随机分为肠内营养组(43例)和对照组(42例);肠内营养组给予肠内营养液(瑞代),对照组给予自制匀浆流食,两组患者均观察3周。采用酶联免疫法测定两组患者GLP-1;采用流式细胞仪检测患者CD3+(总T淋巴细胞)、CD4+(辅助性T细胞)、CD8+(毒性T细胞)和CD4+CD25+Foxp3+Treg细胞(调节性T细胞)。结果治疗后,肠内营养组血浆GLP-1与对照组比较显著升高(t=0.043,P<0.05);治疗后,肠内营养组CD3+、CD4+和CD4+CD25+Foxp3+Treg细胞明显增加(t分别=2.45、2.53、2.01,P均<0.05),GLP-1浓度与毒性T细胞亚群呈负相关(P<0.05)。结论肠内营养支持可提高脑卒中患者血浆中GLP-1浓度,且GLP-1的水平与毒性T细胞呈负相关。
Objective To investigate the effect of enteral nutrition(EN) on plasma glucagon- like peptide- 1 (GLP-1) and cellular immunity in stroke patients. Methods Eighty-five patients with stroke were randomly divid-ed into EN group(n=43) and control group(n=42). Patients in EN group received enteral nutritional emulsion(Fre-subin Diabetes)and patients in control group were on self-made liquid diet,for 3 weeks. The level of GLP-1 was detected by ELISA. The level of CD3+,CD4+,CD8+,and CD4+CD25+Foxp3+Treg was determined by flow cytome-try. Results Compared with control group, patients in EN group had higher level of GLP-1 after treatment (t=0.043,P〈0.05);the level of CD3+,CD4+,and CD4+CD25+Foxp3+Treg was increased in EN group after treatment(t=2.45,2.53,2.01,Ps〈0.05). The level of GLP-1 was negatively related to levels of T cell subsets(P〈0.05). Conclu-sion EN can increase the concentration of plasma GLP-1 in stroke patients. The GLP-1 level is negatively related to toxic T cells.
