亚低温对重型颅脑损伤术中急性脑膨出的影响 被引量:12

摘要 目的:探讨亚低温对重型颅脑损伤标准外伤去骨瓣减压术中急性脑膨出的影响。方法96例重型颅脑损伤患者按随机数字表法分为亚低温组47例和常温组49例,亚低温组术中采用亚低温,监测肛温于33℃~34℃后,行标准外伤大骨瓣开颅减压术及颅内血肿清除术;常温组49例术中未进行亚低温处理,监测肛温36.5℃~38.5℃,行标准外伤大骨瓣开颅减压术及颅内血肿清除术。结果亚低温组术中轻度脑膨出24例,中度脑膨出15例,重度脑膨出8例,常温组分别为9例、24例、16例,亚低温组术中脑膨出程度较低( P<0.01)。术后90 d,亚低温组死亡17例(36.2%)、良好15例(31.9%)、中残8例(17.0%)、重残4例(8.5%)、植物生存3例(6.4%),常温组分别为29例(59.2%)、8例(16.3%)、6例(12.2%)、3例(6.1%)、3例(6.1%),亚低温组预后好于常温组(P<0.05)。结论重型颅脑损伤施行标准外伤大骨瓣开颅减压术中采用亚低温,可降低急性脑膨出的严重程度,改善患者预后。
