甲状腺激素替代治疗对老年甲状腺功能减退患者血脂及心功能的影响研究 被引量:5

摘要 目的研究甲状腺激素替代治疗对老年甲状腺功能减退患者血脂及心功能的影响。方法选择绵阳市中心医院内分泌科2011年9月至2012年9月门诊及住院收治的老年甲状腺功能减退患者作为研究对象,住院后根据甲状腺功能水平给予甲状腺激素替代治疗,检测治疗前后的甲状腺功能指标、血脂相关指标及心功能指标。结果患者治疗后的甲状腺功能指标与治疗前比较,促甲状腺激素[(4.68±1.37)mol/L vs(12.64±4.79)mol/L]水平显著低于治疗前,游离三碘甲状腺原氨酸[(4.75±0.45)pmol/L vs(1.53±0.57)pmol/L]、游离甲状腺素[(1.23±0.18)pmol/L vs(0.57±0.04)pmol/L]水平显著高于治疗前;治疗后的血脂指标与治疗前比较,三酰甘油[(3.46±0.65)mmol/L vs(6.85±0.48)mmol/L]、胆固醇[(1.86±0.43)mmol/L vs(4.54±0.87)mmol/L]、低密度脂蛋白[(2.46±0.54)mmol/L vs(3.54±0.57)mmol/L]水平显著低于治疗前,高密度脂蛋白[(1.54±0.16)mmol/L vs(0.76±0.07)mmol/L]水平显著高于治疗前;治疗后的左心室舒张功能与治疗前比较,E/A值[1.357±0.143 vs 0.754±0.076]、左心室射血分数[(65.45±7.54)%vs(58.75±5.75)%]、左心室舒张末期直径[(54.34±5.57)mm vs(43.54±4.46)mm]均显著高于治疗前,差异均具有统计学意义(P<0.05)。结论甲状腺激素替代治疗能够改善患者的血脂水平及心功能,具有积极的临床价值。 Objective To study the effects of replacement therapy of thyroid hormone on serum lipid and cardiac function in elderly patients with hypothyroidism. Methods Elderly patients with hypothyroidism from outpatient and inpatient Department in Mianyang Central Hospital from Sep. 2011 to Sep. 2012 were studied as research subjects, and were given replacement therapy of thyroid hormone, the indicators of thyroid function, blood lipid indicators and cardiac function before and after treatment were detected. Results After treatment,TSH[ (4.68 ± 1.37 ) mol/L vs ( 12.64 ± 4.79 ) mol/L ± level was significantly lower than that before treatment, FT3 [ (4.75 =t: 0.45 ) pmol/L vs ( 1.53 ± 0.57 ) pmoL/L 1 , FT4 [ ( 1.23 ± 0. 18 ) pmoL/L vs (0.57 +_ 0.04) pmol/L] level,TG [ ( 3.46 ± 0.65 ) mmol/L vs ( 6.85 ± 0.48 ) mmol/L ], TC [ ( 1.86 ± 0.43) mmol/L vs(4.54 20.87) mmol/L] ,LDL[ (2.46 ±0.54) mmol/L vs(3.54 +0.57) mmoL/L] was significantly lower than that before treatment( P 〈 0.05 ) ; HDL[ ( 1.54 ± 0. 16) mmol/L vs ( 0.76 ± 0.07 ) mmol/L] values was significantly higher than that before treatment E/A [ ( 1. 357 ± 0. 143 ) vs (0. 754 ± 0.076)],LVEF[(65.45 +7.54)% vs(58.75 ±5.75)%],LVEDD value[(54.34 ±5.57) mm vs (43.54 ± 4.46 ) mm ] were significantly higher than those before lreatment, with statistically significant difference ( P 〈 O. 05 ). Conclusion Replacement therapy of thyroid hormone can improve blood lipid levels and cardiac function, which is of positive clinical value.
