摘要
目的探讨脐血S100B蛋白对围产儿脑损伤早期诊断和监测的临床价值。方法 233例新生儿,其中阴道分娩窒息组60例,剖宫产窒息组55例,阴道分娩健康组65例,剖宫产健康组53例;比较出生1 min Apgar评分>7分组(213例)与≤7分组(20例)的脐血S100B蛋白水平;对宫内窘迫、窒息的115例围产儿进行影像学检查,确诊为缺氧缺血性脑病(HIE)围产儿75例与非HIE 40例采用酶联免疫吸附试验检测各组脐血S100B蛋白水平,并对结果进行对比分析。结果脐血S100B蛋白水平阴道分娩窒息组为(0.31±0.08)μg/L、剖宫产窒息组为(0.17±0.07)μg/L,明显高于阴道分娩健康组的(0.08±0.03)μg/L及剖宫产健康组的(0.08±0.04)μg/L(P<0.01);出生1 min Apgar评分>7分组脐血S100B蛋白水平为(0.16±0.14)μg/L,≤7分组为(0.20±0.14)μg/L,两者差异无统计学意义(P>0.05);HIE组S100B蛋白水平为(0.30±0.08)μg/L,显著高于非HIE组的(0.13±0.05)μg/L(P<0.01)。结论新生儿脐血S100B蛋白浓度对早期诊断围产儿脑损伤有重要的临床价值。
Objective To investigate the clinical significance of S 100 B protein in umbilical cord blood for early diagnosis and monitoring of perinatal brain injury .Methods All of the 233 neonates were divided into 4 groups . Sixty neonates of vaginal delivery with asphyxia in group A ,55 neonates of cesarean delivery with asphyxia in group B , 65 neonates of vaginal delivery without asphyxia in group C ,and 53 neonates of cesarean delivery without asphyxia in group D.The levels of S100B protein between groups of Apgar score 1 minute after delivery 〉7 and≤7 were compared.Imaging studies were performed in the 115 neonates with intrauterine asphyxia .Enzyme-linked immunosorbent assay ( ELISA ) was used to detect the level of S 100 B protein in umbilical cord blood in 75 neonates with confirmed hypoxic ischemic encephalopathy ( HIE) and 40 neonates without HIE ,and a comparative analysis was performed .Results The levels of S100B protein in umbilical cord blood in group A [(0.31 ±0.08)μg/L],group B[(0.17 ±0.07)μg/L] were significantly higher than those in group C [(0.08 ±0.03)μg/L],group D[(0.08 ±0.04)μg/L,P〈0.01].The levels of S100 B protein showed no significant difference between groups of Apgar score 1 minute after delivery 〉7 and ≤7[(0.16 ±0.14)μg/L vs.(0.20 ±0.14)μg/L,P〉0.05].The level of S100B protein in HIE group was significantly higher than that in non-HIE group[(0.30 ±0.08)μg/L vs.(0.13 ±0.05)μg/L,P〈0.01].Conclusion The detection of S100B protein in perinatal cord blood plays an important role in the early diagnosis of perinatal brain injury.
