胱抑素C对冠心病患者冠状动脉狭窄程度的预测作用 被引量:17

摘要 目的:研究胱抑素C(CysC)与冠状动脉狭窄程度的关系,探讨胱抑素 C 对冠心病患者冠状动脉病变严重程度的预测作用。方法290例患者行冠状动脉造影后对其中确诊冠心病的219例进行冠脉 Gensini 评分,并将其分为低分组(0-30分,n=110例)和高分组(≥30分,n=109例),检测血清胱抑素C水平,应用 SPSS17.0软件进行统计分析。结果冠心病患者血清CysC水平高于非冠心病患者(P〈0.05)。根据冠脉 Gensini积分将患者分组后,高分组 CysC 水平高于低分组(P〈0.05),相关性分析显示CysC水平与 Gensini积分呈正相关(r=0.259,P〈0.01),多因素Logistic回归分析显示高CysC水平和高龄、男性、低 HDL水平是高 Gensini积分的独立危险因素。受试者工作特征曲线(ROC曲线)显示 CysC的曲线下面积为0.729(95%CI:0.612-0.846),当CysC诊断临界值取1.08 mg/L 时,其诊断效率最高,敏感性为77.4%,特异性为67.5%。结论胱抑素C与冠心病严重程度有相关性,可作为预测冠状动脉狭窄程度的指标应用于临床。
