摘要
目的建立适用于临床需求的标本流程监控程序,缩短检验结果回报时间(TAT),提高检验报告及时率。方法分析检验流程中的各时间节点;建立流程中关键节点的信息化监控程序;建立标本进入检验科时间的预警系统;统计分析预警前后的TAT监控效果。结果 (1)临床常规检验流程中共存在12个节点,包括检验医嘱、医嘱处理、医嘱交接、采集标本、转运标本、检验接收、各工作站接收、上机检测、检测结束、报告审核、发送报告及打印报告。(2)流程中检验科需要常规监控的节点有8个,包括采集标本、检验接收、各工作站接收、上机检测、检测结束、报告审核、发送报告及打印报告。(3)医生通过医嘱工作站专设的"标本流向查询"界面实时查询每份标本的流向,降低了检验科的咨询电话的发生率。(4)检验科内部在标本到达TAT的预设值时通过大屏幕显示报警,实现了预警时间前移。"报告延时提醒大屏幕"安装前后检验的平均报告时间明显下降,报告及时率提高。结论通过将临床标本检验周期监控节点前移,提高了检验报告的及时率,有效避免了临床投诉。
Objective To establish a monitor program of specimen process which should be capable to shorten the turnaround time(TAT) of specimens,promote the timeliness rate of report and be suited for the requirement of clinical practice. Methods The each time point of test process was analyzed. The informationized monitor program for the key points of the whole test process was established. The early warning system for the time when each specimen arrived in the clinical laboratory was set up. The effects of TAT monitoring before and after early warning were statistically analyzed. Results 1. There were 12 time points in the routine examination process,including medical orders for examination,disposition of medical orders,notification of medical orders,collection of specimens,delivery of specimens,acceptance of specimens,reception of specimens in workstation,determination of specimens in instruments,termination of determination,report checking,report sending and report printing. 2. The 8 time points which should be monitored by clinical laboratory were collection of specimens,acceptance of specimen,reception of specimens in workstation,determination of specimens in instruments,termination of determination,report checking,report sending and report printing. 3. The details of the process of each specimen could be viewed in the option of " The query of sample location" through the medical workstation,by which the inquiries of telephone to clinical laboratory considerably reduced. 4. The countdown warnings were appeared on the large screen of laboratory to remind the technicians when the default of TAT for a sample appeared,thus the warning was achieved in advance. The average report time of examination was shortened significantly following the installation of the large screen of " Report delay reminding",and the timeliness rate of reports remarkably improved. Conclusion The timeliness rate of reports in clinical laboratories was significantly improved by monitoring TAT of specimen in advance and the complaints from clinical departments were effectively avoided.
