摘要
目的:应用超声血管回声跟踪技术(ET)检测高血压患者治疗前后颈总动脉弹性的变化,分析高血压患者血压控制水平不同对颈总动脉弹性改善程度的影响,探讨ET对临床治疗高血压及预防动脉粥样硬化的指导价值。方法选取2010年1月-2012年1月就诊于河北联合大学附属医院的高血压患者234例,应用降压药物进行个体化治疗,血压≤140/90 mm Hg (1 mm Hg=0.133 kPa),持续3个月。应用ET检测高血压患者治疗前后颈总动脉弹性指标:僵硬度(β)、压力应变弹性系数( Ep)、顺应性( AC)、增大指数( AI)及脉搏波传导速度( Pwvβ)。分析不同收缩压下降水平(下降0~10 mm Hg、下降11~20 mm Hg、下降21~30 mm Hg及下降>30 mm Hg)和不同舒张压下降水平(下降0~10 mm Hg、下降11~20 mm Hg及下降>20 mm Hg)高血压患者颈总动脉弹性指标的变化。结果高血压患者治疗后较治疗前颈总动脉β、 Ep、 Pwvβ降低, AC升高( P<0.05)。治疗后不同收缩压下降水平高血压患者颈总动脉β、 Ep、 AC、 AI、 Pwvβ变化比较,差异均有统计学意义( P<0.05);不同舒张压下降水平高血压患者颈总动脉β、 Ep、 AI、 Pwvβ变化比较,差异均无统计学意义(P>0.05); AC变化比较差异有统计学意义(P<0.05)。收缩压下降水平与颈总动脉β、 Ep、 AI、 Pwvβ均呈正相关(P<0.05),与AC呈负相关(P<0.05)。舒张压下降水平与颈总动脉β、 Ep、 AC、 AI、 Pwvβ均不具有直线相关性(P>0.05)。结论 ET能清楚检测高血压患者治疗前后颈总动脉弹性变化情况;治疗后不同收缩压下降水平各弹性指标改善情况也不同。 ET指标的变化能指导临床治疗高血压,早期预防动脉粥样硬化的发生。
ObjectiveToanalyzethechangeofcarotidarteryelasticityaftertreatmentbyechotracking(ET)inhy-pertension patients , and to analyze the influence of hypertension level on the improvement of carotid artery elasticity , as well as tostudythevalueofETinthetreatmentofhypertensionandpreventingatherosclerosis.Methods 234hypertensionpatientsad-mitted to the Hospital Affiliated to Hebei United University from January 2010 to January 2012 were given individualized treatment for three consecutive months , with the blood pressure≤140/90 mm Hg (1 mm Hg=0.133 kPa) .ET was used to detect elastic parameters of β, Ep, AC, AI and Pwvβ.The change of carotid artery elasticity in patients with different decreasing levels of sys -tolic blood pressure (0-10 mm Hg, 11-20 mm Hg, 21-30 mm Hg and 〉30 mm Hg) and diastolic blood pressure (0-10 mm Hg, 11-20 mm Hg and 〉20 mm Hg) .Results The β, Ep and Pwvβwere decreased and the AC was increased after treatment ( P〈0.05) .Theβ, Ep, AC, AI and Pwvβall showed statistically significant differences in patients with different decreasing levels of systolic blood pressure ( P〈0.05) .Theβ, Ep, AI and Pwvβdidn′t show statistically significant difference in patients with different decreasing levels of diastolic blood pressure ( P〉0.05 ) , but AC showed statistically significant differ-ence ( P〈0.05 ) .The decreasing level of systolic blood pressure was positively correlated with β, Ep, AI and Pwvβ( P〈0.05), but was negatively correlated with AC (P〈0.05) .The decreasing levels of diastolic blood pressure was not correlated withβ, Ep, AC, AI and Pwvβ( P〉0.05) .Conclusion ET can accurately detect the changes of carotid artery elasticity in patients with hypertension .The elastic parameters are improved differently in accordance with different levels of systolic blood pres -sure.The changes of ET parameters can guide clinical treatment of hypertension and prevent atherosclerosis in an early stage .
