摘要 目的探讨磁共振快速STIR和钆增强技术在成人早期股骨头缺血性坏死(avascular necrosis of femoral head in adult,ANFH)诊断的价值。方法回顾性分析44例早期ANFH的MRI表现。检查序列分别为:T1WI、T2WI、快速STIR和钆增强扫描等。结果 44例ANFH中,0期8髋(左侧5髋、右侧3髋),I期14髋(左侧6髋、右侧8髋),II期26髋(左侧12髋、右14髋)。双线征占56.25%,骨髓水肿占33.33%,关节积液占43.75%,软骨破坏占25%。结论快速STIR序列对ANFH诊断较SE序列更为敏感,钆增强扫描能准确区别坏死与存活组织,因此这两种技术对早期ANFH诊断更精确。 Objective To evaluate Fast-STIR and Gd-DTPA-enhanced techniques in diagnostic value of MRI in early stage of avascular necrosis of femoral head in adult. Methods This was a retrospective study on MRI of 44 cases in early stage of avascular necrosis of femoral heads. Examination sequences included:T1WI , T2WI, Fast-STIR , Gd-DTPA-enhanced. Results In 44 cases, 8 hips(left 5 hips, right 3 hips) were in stage 0, 14 hips(left 6 hips, fight 8 hips) were in stage I, 26 hips(left 12hips, fight 14hips) were in stage II. Doule-line sign were appeared in 56.25% of cases, bone marrow edema in 33.33%, joint effusion in 43.75%, cartilage destroy in 25%. Conchicion Fast-STIR sequence was more sensitive than SE sequence. Gadolinium-DTPA-enhanced scanning can identified the tissue of putrescence and living. So the two techniques can diagnose avascular necrosis of femoral head in the early stage.
