二维斑点追踪成像技术评价糖尿病患者左心室功能早期改变 被引量:3

Assessment of early change of left ventricular function in type 2 diabetes mellitus using two-dimensional speckle tracking imaging

摘要 目的应用二维斑点追踪成像(STI)技术检测2型糖尿病患者左心室收缩功能的早期改变,并探讨其临床应用价值。方法 2型糖尿病患者(左心室射血分数≥50%)60例,分为单纯糖尿病组(A组)30例和糖尿病合并左心室壁肥厚组(B组)30例;正常对照组30例。分别测量左心室长轴及短轴各节段收缩期纵向、径向、圆周峰值应变,并计算其平均值。结果与对照组相比,糖尿病组收缩期纵向、圆周峰值应变及平均应变均明显减低(P<0.05),B组低于A组(P<0.05)。与对照组比较,糖尿病组径向峰值应变及平均应变亦减低,但差异无统计学意义(P>0.05)。结论 STI技术可早期检测糖尿病患者左心室心肌局部收缩功能异常。 Objective To investigate the clinical value of two-dimensional speckle tracking imaging in assessment of early change of left ventricular (LV) systolic function of type 2 diabetes mellitus (T2DM). Methods The study included 30 T2DM with diabetes mellitus only (groups A), 30 T2DM with both diabetes and left ventricular hypertrophy (groups B), and 30 normal controls. The LV systolic peak longitudinal, radial, circumferential strain of each plane were measured, and average peak strain were assessed. Results Peak longitudinal, circumferential strain and average peak strain in both T2DM groups (A, B) were significantly reduced than that of controls (P〈0.05), and those in group B were lower than in group A (P〈0.05). There were no significant difference of peak radial strain and average peak strain among the three groups(P〉0.05). Conclusion Two-dimensional speckle tracking imaging could be used to identify the early change of LV local systolic function in T2DM.
