摘要
目的探寻18F-FDG PET/CT在诊断脊椎结核中的应用价值及影像学表现。方法回顾性分析经手术病理证实的20例脊椎结核患者的资料,所有患者术前均行MRI和PET/CT检查,对比分析两者间的检出率及诊断准确率。结果 20例中,MRI检出36个椎体,诊断脊柱结核12例(60﹪)。18F-FDG PET/CT检出36个椎体,其中4例为单一椎体受累,16例表现为2个连续椎体同时受累,以胸腰段椎体为主,诊断脊椎结核20例(100﹪)。PET/CT和MRI病灶检出率相同,均为100﹪,定性诊断正确率分别为100﹪和60﹪,PET/CT定性诊断率高于MR,差异有统计学意义。结论 PET/CT在诊断脊椎结核方面优于MRI,两者结合能为临床诊治提供更准确的影像学信息。
Objective To explore the potential value of ^18F-FDG PET/CT application in the diagnosis of spinal tuberculosis. Methods A retrospective analysis was conducted in 20 cases of pathologically confirmed spinal tuberculosis. MRI and PET/CT images acquired before surgery were compared in the detection rate and diagnosis accuracy. Results Among 20 cases, 36 affected vertebrae were detected by MRI, resulting in the diagnosis of spinal tuberculosis in 12 cases(60 ﹪). 36 affected vertebrae were detected by ^18F-FDG PET/CT, including 4 cases with single affected vertebrae and 16 cases with two consecutive affected vertebrae, majority of which were thoracic lumbar vertebrae, resulting in adiagnosis of spinal tuberculosis in 20 cases(100 ﹪). PET/CT and MRI both have a lesion detection rate of 100 ﹪, qualitative diagnostic accuracy were 100 ﹪ and 60 ﹪respectively, the difference was statistically significant. Conclusions PET/CT is superior to MRI in diagnosis of spinal tuberculosis, a combination of them can provide more accurate imaging information for clinical diagnosis and treatment.
