Investigation on influencing factors of cluster onset of hand- foot- and- mouth disease in kindergartens of Pudong New Area in Shanghai city

摘要 目的:对上海市浦东新区手足口病聚集性发病的托幼机构进行调查,探讨托幼机构手足口病聚集性发病的影响因素。方法:选取2012年10月-2013年12月,在该辖区20个社区发生手足口病聚集性病例的托幼机构为病例组。按幼儿园性质进行1∶1配对选择同一社区内未发生聚集性病例的托幼机构为对照组进行研究。采用条件Logistic回归分析探讨托幼机构手足口病聚集性发病影响因素。结果 :保育员消毒正确性与托幼机构手足口病聚集性发病呈正相关,OR(95%CI)为21.668(1.548~303.285)。结论:保育员消毒正确性为该区托幼机构手足口病聚集性发病的重要影响因素。开展托幼机构手足口病聚集性发病影响因素调查分析,为制定新的托幼机构传染病预防控制与消毒隔离规范提供科学依据,控制聚集性病例的发生。 Objective: To investigate the kindergartens with cluster onset of hand - foot - and - mouth disease (HFMD) in Pudong New Area in Shanghai city, explore the influencing factors of cluster onset of HFMD in kindergartens. Methods: The kindergartens with cluster onset of HFMD were selected as case group from 20 communities in Pudong New Area from October 2012 to December 2013; while the kindergartens without cluster onset of HFMD were selected as control group from the same communities according to 1:1 matched principle. Conditional logistic regression analysis was used to explore the influencing factors of cluster onset of HFMD in kindergartens. Results: Correctness of disinfection in nursery governess was positively correlated with cluster onset of HFMD in kindergartens ( OR = 21. 668, 95% CI: 1. 548 - 303. 285 ) . Conclusion: Correctness of disinfection in nursery governess is one significant influencing factor of cluster onset of HFMD in kindergartens of Pudong New Area in Shanghai city. Conducting investigation and analysis on influencing factors of HFMD in kindergartens, providing a scientific basis for making new prevention, control, disinfection and isolation standards of infectious diseases in kindergartens can control the occurrence of cluster onset of HFMD.
