摘要
目的通过回顾性分析,为临床上在缺乏病理资料时更准确地判断糖尿病肾病患者是否合并非糖尿病肾病提供依据,从而为此类患者的诊断与治疗提供指导。方法回顾性分析2000年2月-2014年2月我院糖尿病肾病患者怀疑合并非糖尿病肾病并接受肾活检的患者共108例。根据肾活检病理资料,将所有患者分为单纯糖尿病肾病组,糖尿病肾病合并非糖尿病肾病组和非糖尿病肾病组。通过分析临床参数预测糖尿病肾病的可靠性。结果糖尿病视网膜病变对于糖尿病肾病的敏感性为57.5%,特异性为88.9%;血尿对于非糖尿病肾病的敏感性为71.2%,特异性为90.6%。和有糖尿病肾病患者相比,非糖尿病肾病患者糖尿病病史明显更短。结论临床资料能较准确提示糖尿病肾病患者合并有非糖尿病肾病,积极筛查此类患者并进行肾活检,尽早明确诊断,有助于更好地指导治疗,从而改善患者预后。
Objective The aim of this analysis was to evaluate how to differentiate non-diabetic renal disease（NDRD）in patients with diabetes mellitus（DN）before kidney biopsy.Methods We retrospectively reviewed the medical records of patients with diabetes who underwent kidney biopsy on clinical suspicion of NDRD at the Suining Central Hospital.According to the pathologic diagnosis,patients were grouped as Group-I,NDRD;Group-II,NDRD with DN;and Group-III isolated DN.By analyzing the clinical parameters we predict the reliability of diabetic nephropathy.Results The result showed that the presence of diabetic retinopathy predicts DN（Sensibility：57.5%,Specificity：88.9%）.Hematuria predicts NDRD（Sensibility：71.2%,Specificity：90.6%）.A shorter duration of diabetes mellitus（DM）also predicted NDRD.Conclusion Our study suggests that special clinical characteristics indicate NDRD in patients with type 2diabetes mellitus.The renal biopsy in those patients helps to diagnose and treat NDRD.
