视黄醇结合蛋白、胱抑素C及β_2微球蛋白联检对2型糖尿病早期肾损害的诊断价值 被引量:4

摘要 目的:探讨视黄醇结合蛋白(RBP)、胱抑素C(Cys C)及β2微球蛋白(β2-MG)联检对2型糖尿病(T2DM)早期肾损害的诊断价值。方法:将选取的125例T2DM患者根据尿白蛋白排泄率分成正常蛋白尿组(62例)、微量蛋白尿组(63例),另选健康体检人员45例做对照组,所有受检者血清RBP采用免疫透射比浊法检测,Cys C、β2-MG采用颗粒增强免疫透射比浊法检测。结果:微量蛋白尿组血清RBP、Cys C、β2-MG、Urea及Crea含量及阳性率较另外两组高(P<0.05);正常蛋白尿组RBP、Cys C及β2-MG含量及阳性率较对照组高(P<0.05);两试验组RBP、Cys C、β2-MG阳性率较Urea或Crea高(P<0.05);两试验组RBP与Cys C联检的阳性率较单项指标高,RBP、Cys C与β2-MG三项联检时阳性率较RBP与Cys C两项联检高(P<0.05)。结论:RBP、Cys C及β2-MG可作为T2DM早期肾损伤诊断的有价值的指标,联合检测意义更大。 Objective:To study the diagnostic value of srum retinol binding protein(RBP),cystatin C(Cys C) andβ2 microglobulin(β2-MG) in early renal damage.Method:125 case of T2DM were selected.The urinary albumin excretion rate(UAER) of 62 Patients with T2DM was less than 20μg/min was normal albuminuria group,the UAER of 63 patients with T2DM was situated between 20μg/min and 200μg/min was microalbuminuria group.45 cases of healthy people was control group.RBP was tested by the immune turbidimetric method,Cys C andβ2-MG were test by particle enhanced immuno turbidimetric assay. Result:The content and the positive rate of RBP,Cys C,β2-MG、ureaand creatinine in microalbuminuria group were higher than other two groups(P〈0.05).The Content and the positive rate of RBP,Cys C,β2-MG in normal albuminuria group were higher than control group(P〈0.05).The positive rate of RBP and Cys C was higher than RBP or Cys C(P〈0.05).The positive rate of RBP,Cys C orβ2-MG in the two experimental group were higher than Urea or Crea(P〈0.05).The positive rate of RBP,Cys C andβ2-MG in the two experimental group were higher than RBP and Cys C(P〈0.05).Conclusion:RBP,Cys C andβ2-MG can be used as marker in diagnosis type 2 diabetes mellitus early renal damage,the clinical value of combined detection is greater.
