摘要
目的：探讨千山活血膏联合补肾通痹酊超声透入治疗膝关节骨性关节炎的疗效.方法：回顾性分析2013年4月～2014年4月膝关节骨性关节炎患者150例,分为超声透入组、膏药组和联合组,每组50例.超声透入组患者平均年龄（56.58±5.47）岁;病程（7.35±2.59）个月;左膝18例,右膝17例,双膝15例;给予补肾通痹酊超声透入.膏药组患者平均年龄（56.25±6.35）岁;病程（7.68±2.76）个月;左膝16例,右膝20例,双膝14例;给予千山活血膏外敷.联合组患者平均年龄（55.65±4.49）岁;病程（7.50±3.36）个月;左膝16例,右膝18例,双膝16例;给予补肾通痹酊超声透入配合千山活血膏外用.治疗后采用Lysholm和Gillquist膝关节评分标准和中医病症诊断疗效标准对3组治疗效果及膝关节功能进行评价.结果：纳入统计的患者148例,其中超声透入组50例,膏药组49例,联合组49例.治疗后联合组治愈37例,好转9例,未愈3例;外敷组治愈25例,好转16例,未愈8例;超声透入组治愈23例,好转16例,未愈11例.联合组的疗效高于超声透入组和膏药组.联合组除交锁和不稳定外,其余各项评分均高于超声透入组;联合组在跛行、疼痛、支撑、肿胀和爬楼梯方面评分高于膏药组;膏药组在疼痛、支撑、爬楼梯和下蹲方面评分高于超声透入组.结论：补肾通痹酊超声透入配合千山活血膏治疗膝关节骨性关节炎可以减轻疼痛,消除肿胀,改善关节功能.千山活血膏能在一定程度上减轻疼痛,改善关节功能.
Objective： To investigate the efficacy of Qianshan Huoxue plaster combined with Bushen Tongbi tincture Ultrasonicpenetration in the treatment of knee osteoarthritis.Methods： A retrospective analysis from April 2013 to April 2014 ,150 cases of kneeosteoarthritis patients, divided into the Ultrasonic penetration group, plaster group and combination group, 50 cases each group. Theaverage age of Ultrasonic penetration group were （56.58 ± 5.47） years old; course of disease （7.35 ± 2.59） months; 18 cases of left, rightknee in 17 cases, 15 cases of double knee; given Bushen Tongbi tincture Ultrasonic penetration. The average age of plaster group were（56.25 ± 6.35） years old; course of disease （7.68 ± 2.76） months; 16 cases of left, right knee in 20 cases, double genu in 14 cases; giveQianshan Huoxue plaster. The average age of patients in the combination group （55.65 ± 4.49） years old; course of disease （7.50 ± 3.36）months; 16 cases of left, right knee in 18 cases, 16 cases of double knee; given Bushen Tongbi tincture Ultrasonic penetration combinedwith Qianshan Huoxue plaster. After treatment with standard Lysholm and Gillquist knee score standard and Chinese disease diagnosis andefficacy evaluation of the therapeutic effect of 3 groups of knee function. Results： 148 cases of patients included in the statistics, theultrasound penetration into the group of 50 cases, 49 cases of plaster group, combined group 49 cases. Combined treatment group, 37 caseswere cured, 9 cases improved, 3 cases was not; treatment group 25 cases were cured, 16 cases improved, 8 cases ineffective; phonophoresisgroup 23 cases were cured, 16 cases improved, 11 cases ineffective. The efficacy of the combined treatment group was higher than that ofUltrasonic penetration group and plaster group. Combined group in addition to the lock and unstable, the scores were higher than theultrasonic penetration group; in the combined group limp, pain, swelling and support, stair climbing score higher than plaster; plastergroups in pain, support, climb the stairs and squat score higher than ultrasonic penetration group. Conclusion： Bushen Tongbi tincturephonophoresis combined with Qianshan Huoxue plaster in the treatment of knee osteoarthritis can relieve pain, eliminating swelling,improve joint function. Qianshan Huoxue plaster can reduce the pain to a certain extent, improve joint function.
