摘要
目的探讨翼状胬肉切除联合自体结膜瓣移植手术中2种不同的缝合方法对泪膜稳定性的影响。方法68例行翼状胬肉切除联合自体结膜瓣移植手术患者根据缝合方法不同分为2组,间断缝合组31例(31眼)采用间断缝合法;连续锁边缝合组37例(37眼)采用连续锁边缝合法;术后给予抗感染治疗。比较2组患者手术前及术后1、3个月的泪膜破裂时间(BUT),评价各个时期的泪膜稳定性。结果 2组患者术后早期均有不同程度的局部刺激症状,连续锁边缝合组患者疼痛及异物感等刺激症状较轻。2组患者术前BUT比较差异无统计学意义(P>0.05)。2组患者术前BUT与术后1个月比较差异均无统计学意义(P>0.05),2组患者术后1个月BUT与术后3个月比较差异均无统计学意义(P>0.05),2组患者术后3个月BUT长于术前,差异均有统计学意义(P<0.05)。术后1、3个月,2组患者BUT比较差异均无统计学意义(P>0.05)。结论在翼状胬肉切除联合自体结膜移植术中,间断缝合法和连续锁边缝合法均可改善术后患者的泪膜稳定性,但采用连续锁边缝合法的患者术后刺激症状较轻。
Objective To investigate the effect of two different suture methods on tear film stability in pterygium excision combined with autologous conjunctival flap graft operation. Methods A total of 68 patients with pterygium who underwent pterygium excision and autologous eonjunctival flap graft operation were divided into two groups according to the suture method. Thirty-one patients（31 eyes） in intermittent suture group were treated with intermittent suture;thirty-seven patients （37 eyes） in continuous side whipstitch suture group were treated with continuous side whipstitch suture. All patients received antiinfective therapy after operation. The breakup time of tear film（BUT） was compared between the two groups at the time point of before treatment, 1 month and 3 months after treatment. The tear film stability was evaluated at different time. Results All patients had varying degree of early local irritation after operation in the two groups, but the pain and foreign body sensation were mild in patients of continuous side whipstitch suture group. There was no significant difference in BUT between the two groups before operation（P 〉 0.05 ）. There was no significant difference in BUT before operation and one month after operation between the two groups （ P 〉 0.05 ）. There was no significant difference in BUT between the two groups one month and three months after operation （ P 〉 0.05 ）. Three months after operation, the BUT was longer than that before operation in the two groups（P 〈0.05 ）. One month and three months after operation, there was no significant difference in BUT between the two groups（P 〉 0.05 ）. Conclusions Intermittent suture and continuous side whipstitch suture in pterygium excision combined with autologous eonjunctival flap transplantation operation can significantly improve the tear film stability. But the patients with continuous side whipstitch suture have milder stimulation symptoms after operation.
