摘要
目的观察胃十二指肠良性疾病患者行胃大部切除术后的营养状况及生存质量水平。方法选择2005年3月至2012年3月在驻马店市中心医院因胃十二指肠良性疾病行胃大部切除术患者223例作为观察组,并选择同期健康体检者200例作为对照组。观察2组受检者的生物化学指标、免疫指标、贫血及骨质疏松情况,并对2组受检者的生存质量评分进行比较。结果观察组患者血清维生素B12、叶酸、白蛋白、铁蛋白、血红蛋白水平显著低于对照组,差异均有统计学意义(P<0.05,P<0.01)。对照组和观察组受检者贫血发生率分别为7.0%和22.0%,观察组患者贫血发生率显著高于对照组,差异有统计学意义(P<0.01)。观察组患者血清碱性磷酸酶水平显著高于对照组,差异有统计学意义(P<0.01)。2组受检者血清钙、磷、镁水平比较差异均无统计学意义(P>0.05)。对照组和观察组受检者骨质疏松发生率分别为9.5%和30.5%,观察组患者骨质疏松发生率显著高于对照组,差异有统计学意义(P<0.01)。对照组和观察组受检者的生存质量评分分别为122.6±7.5和97.9±13.3,观察组患者的生存质量评分显著低于对照组,差异有统计学意义(P<0.05)。结论胃十二指肠良性疾病患者行胃大部切除术后的营养状况及生存质量均显著下降。
Objective To explore the nutritional status and quality of life of patients with gastroduodenal benign disease after subtotal gastrectomy. Methods A total of 223 patients with gastroduodenal benign disease who underwent subtotal gastrectomy in Zhumadian Central Hospital from March 2005 to March 2012 were selected as observation group, and 200 healthy people were selected as control group in the same time. The biochemical parameters, immunological parameters, anaemia and bone rarefaction were observed in the two groups. The score of quality of life was compared between the two groups. Results The levels of serum vitamin B12, folacin, albumin, ferritin and hemoglobin in observation group were significantly lower than those in control group(P 〈0.05 ,P 〈0.01 ). The anaemia rate in control group and observation group was 7.0% and 22.0%, respectively ;the anaemia rate in observation group was significantly higher than that in control group ( P 〈 0. 01 ). The serum serum alkaline phosphatase level in observation group was significantly higher than that in control group (P 〈 0. 01 ). There was no significant difference in the level of serum calcium, phosphorus and magnesium between the two groups ( P 〉 0.05 ). The bone rarefaction rate in control group and observation group was 9.5% and 30. 5%, respectively;the bone rarefaction rate in observation group was significantly higher than that in control group ( P 〈 0.01 ). The score of quality of life in control group and observation group was 122.6±7.5 and 97.9±13.3 ,respectively;the score of quality of life in observation group was significantly lower than that in control group ( P 〈 0.05). Conclusion The nutritional status and quality of life are significantly decreased in patients with gastroduodenal benign disease after subtotal gastrectomy.
