摘要 目的:探讨结节性甲状腺肿(Nodular Goiter,NG)的MSCT(Multiple slice CT)表现。旨在提高对本病的认识。材料与方法:前瞻性地收集61例拟诊结节性甲状腺肿患者完整资料,所有患者在MSCT扫描后72小时内手术。术前就与结节性甲状腺肿相关的5种MSCT征象双盲软读片,分别计算其诊断的灵敏度,特异度及总正确率。阅片结论与手术病理对照,采用配对四格表,运用Fisher确切概率法χ2检验。结果:本组资料研究显示术前运用主要MSCT征象对结节性甲状腺肿总诊断正确率78.7%(48/61);结节性甲状腺肿的MSCT表现为蛋壳样钙化影、葡萄串样影、月晕影,环形强化影,包膜完整影,其灵敏度分别为6.3%、89.6%、54.2%、8.3%和93.4%;特异度分别为100%、92.3%、53.8%,76.9%和84.6%。MSCT对结节性甲状腺肿诊断与病理诊断没有显著性差异(χ2=2.78,P=0.096)。结论:结节性甲状腺肿MSCT表现具有一定的特征,术前MSCT扫描可对结节性甲状腺肿做出准确诊断。 Purpose: To explore the MSCT( Multiple slice CT) manifestations of Nodular Goiter,and to evaluate its diagnosis of this disease. Materials and Methods: We studied 61 consecutive patients on MSCT who had pathologic proven nodular goiter 45 of 61. All patients were performed surgery within 72 hours after MSCT. Two experienced radiologists,blinded to the pathologic diagnosis,reached a diagnosis from the five MSCT findings. MSCT diagnosis data and pathologically proved were analyzed and compared by Fisher probabilities χ^2test. Results: Our study indicate that MSCT images was predictive of the diagnosis of Nodular Goiter in 78. 7%( 48 /61) patients. The findings of nodular goiter mainly consist of " eggshell" calcification imaging,bunches of grapes imaging,lunar halo,circular enhancement and complete envelope of thyroid imaging. Those sensitivity were 6. 3% 、89. 6% 、54. 2% 、8. 3% and 93. 4%, and specificity were 100% 、92. 3% 、53. 8%,76. 9% and 84. 6% respectively. There was no significant between MSCT and pathologic diagnosis of nodular goiter( χ^2= 2. 78,P = 0. 096). Conclusion: The MSCT manifestations of NG were some characteristics. MSCT examination was of significant value in nodular goiter before operation.
