胎心监护联合肾动脉、大脑中动脉及静脉导管分流率预测胎儿宫内窘迫的临床分析 被引量:7

摘要 目的分析胎心监护联合肾动脉、大脑中动脉及静脉导管分流率预测胎儿宫内窘迫的临床价值。方法选择本院收治的68例胎儿宫内窘迫孕妇为观察组,另选择同期于本院产科分娩的60例正常孕妇为对照组,分别对两组孕妇的胎心率及肾动脉和大脑中动脉RI、PI、S/D值及静脉导管(DV)分流率进行检查,对比分析两组孕妇各指标值及各指标预测胎儿宫内窘迫的阳性率。结果观察组胎儿的肾动脉RI、PI、S/D值均高于对照组(P<0.05),大脑中动脉RI、PI、S/D值均低于对照组(P<0.05),DV分流率高于对照组(P<0.05)。观察组胎儿的胎心率以及肾动脉和大脑中动脉RI、PI、S/D值及DV分流率预测胎儿宫内窘迫的阳性率均高于对照组(P<0.05),且4项联合预测胎儿宫内窘迫的阳性率均高于各项单独预测的阳性率(P<0.05)。结论胎心监护联合肾动脉、大脑中动脉及DV分流率在胎儿宫内窘迫预测中具有良好的临床应用价值。 Objective To analyze the clinical value of fetal heart rate monitoring combined with renal artery,middle cerebral artery and ductus venosus shunt rate in the prediction of fetal distress. Methods Sixty-eight pregnant women with fetal distress admitted to our hospital were selected as the observation group retrospectively and 60 normal preg-nant women who delivered in the obstetrical department of our hospital in the same period were selected as the control group.The fetal heart rates,RI,PI and S/D values of renal artery and middle cerebral artery and ductus venosus shunt rates (DV shunt rates) of the two groups were examined respectively.The indicators values and the positive rates of indi-cators in predicting fetal distress of the two groups were compared and analyzed. Results The observation group had higher RI,PI and S/D values of fetal renal artery than the control group (P〈0.05),lower RI,PI and S/D values of middle cerebral artery than the control group (P〈0.05),and higher DV shunt rate than the control group (P〈0.05).The positive rates of the fetal heart rate,RI,PI and S/D values of renal artery and middle cerebral artery and ductus venosus shunt rate in predicting fetal distress of the observation group were all higher than those of the control group (P〈0.05),and the positive rate of the four indicators joined together was higher than each single indicator in predicting fetal distress (P〈0.05). Conclusion Fetal heart rate monitoring combined with renal artery,middle cerebral artery and ductus venosus shunt rate has good clinical application value in the prediction of fetal distress.
