摘要
目的:通过对活体猪胫骨行经皮射频消融(PRFA),观察集束单电极RFA对胫骨骨端和骨干的损毁情况,为骨肿瘤的临床RFA治疗提供参考。方法:活猪16头,麻醉状态下,于两侧后腿胫骨骨端、骨干的相同位置行RFA,RFA后立即处死实验猪,获取的标本肉眼观察标本、制成切片后光镜下观察并测量。结果:骨端骨松质RFA灶坏死区的三维形态是长轴与针道重叠的椭球体,长轴(2.59±0.21)cm,短轴(2.15±0.10)cm,体积(6.34±0.27)cm3。骨干髓腔RFA灶坏死区的三维形态是长轴与骨干长轴平行而与针道垂直的椭球体,长轴(4.53±0.20)cm,短轴(1.71±0.22)cm,体积(7.14±0.36)cm3。统计分析显示,骨端骨松质及骨干骨髓组织RFA灶坏死区体积均小于射频消融仪标定坏死区面积(P均小于0.05),骨端骨松质小于骨干骨髓组织RFA灶坏死体积(t=2.011,P=0.049﹤0.05)。结论:RFA能够有效损毁骨端骨松质及骨干骨髓组织,可以应用于骨肿瘤的临床治疗。
Purpose:To observe the damaged situation of lesions created in tibias of the swine limbs in vivo with radiofrequency,and to provide references to clinical application of radiofrequency ablation in bone tumors.Methods:Sixteen swans were performed with RFA in the same position of tibial epiphysis and bone shaft under anesthesia with DSA guidance.They were killed after the RFA was finished,and the epiphysis and bone shaft specimens were got.The form of radiofrequency ablation focal and the boundary of the necrosis were observed.Results:First,the three-dimensional shape of necrotic area located in the cancellous bone of the upper end after RFA was ellipsoid,whose long axis was superimposed on the needle passage of electrode needle.Its long axis was (2.59±0.21) cm,its short axis was (2.15±0.10) cm,and its volume was (6.34±0.27) cm3.The three-dimensional shape of necrotic area located in diaphysis after RFA was ellipsoid whose long axis was perpendicular to the needle passage of electrode needle and parallel to the longitudinal axis of diaphysis.Its long axis was (4.53 ±0.20) cm,its short axis was (1.71±0.22) cm,and its volume was (7.14±0.36) cm3.Secondly,the volume of necrotic area located in the cancellous bone of the upper end and diaphysis was less than standardization data about necrotic area from RFA system,and the volume of necrotic area located in the cancellous bone of the upper end was less than that of diaphysis.Conclusion:RFA can effectively damage bone which located in the tibias of the swine in vivo,RFA can be applied to the treatment of bone tumors.
