PCOS合并胰岛素抵抗的血脂代谢异常特点分析 被引量:8

摘要 目的探讨和分析多囊卵巢综合征(PCOS)患者血脂代谢异常及与胰岛素抵抗(IR)的相关性。方法回顾分析了该院128例多囊卵巢综合征的临床资料,根据胰岛素抵抗指数(HOMA-IR)将128例患者分为两组,胰岛素抵抗组72人和非胰岛素抵抗组56人,分析比较了IR组和NIR组的各项生理生化指标,两组各自的血脂异常发生率以及血脂四项指标与其他生理生化指标的偏相关性分析。结果 IR组中的患者的BMI、WHR、SBP、DBP、IL-18、TC、TG和LDL测量值明显比NIR组中患者要高,IR组中的患者的LH/FSH、DHEAS和HDL的测量值明显比NIR组中患者要低,差异具有显著性(P<0.05);IR组患者的血脂异常发生率明显高于NIR组(45%VS 14%,25%VS 3%,30%VS 5%,15%VS 2%),差异具有显著性(P<0.05);总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)水平与HOMA-IR值呈显著正相关关系(r=0.321,P=0.001;r=0.097,P=0.005;r=0.089,P=0.013);低密度脂蛋白(LDL)水平与HOMA-IR值呈显著负相关关系(r=-0.124,P=0.003)。结论 PCOS患者胰岛素抵抗和血脂异常的发生率均较高,血脂异常与胰岛素抵抗具有相关性。 【Objective】To investigate the prevalence of dyslipidemia in women with polycystic ovary syndrome(PCOS) and its relationship to insulin resistance(IR).【Method】A retrospective study was performed in 128 PCOS patients who were divided into two groups, IR group and NIR group according to the HOMA-IR. The incidence of dyslipidemia were compared in two groups. Lipids four indicators with other physiological and biochemical were analyzed of partial correlation.【Results】The value of BMI, WHR, SBP, DBP, IL-18, TC, TG and LDL in the IR group was higher than that in the NIR group. The value of LH/FSH, DHEAS and HDL in the IR group was lower than that in the NIR group. The difference was statistically significant between the two groups(P〈0.05). The rate of dyslipidemia in the IR group was higher than that in the NIR group(45% VS 14%, 25% VS 3%, 30% VS 5%, 15% VS2%). Homeostasis model assessment(HOMA) index was found positively correlated with TG, TC and LDL(r =0.321,P =0.001; r =0.097, P =0.005; r =0.089, P =0.013), but negatively correlated with HDL(r =-0.124, P =0.003).【Conclusion】Both of the prevalence of IR and dyslipidemia were found high in PCOS women in this study. And the dyslipidemia in women with PCOS was correlated with insulin resistance(IR).
