摘要
目的 调查分析湖南邵阳地区医院2012~2013年临床分离肺克雷伯菌(KPN)的药物敏感性,探讨氨基糖苷乙酰转移酶基因(AAC)在耐氨基糖苷类药KPN菌株中的携带情况.方法 收集邵阳地区新邵县人民医院,洞口县人民医院,邵阳医专附属医院三家医院2012年8月~2013年12月临床连续分离非重复KPN 259株,梅里埃药敏条和KB纸片法检测其药物敏感性.采用PCR法对氨基糖苷类修饰酶(AMEs) AAC(3)-Ⅰ,AAC(6')-Ⅰb,AAC(3")-Ⅰ,AAC(3')Ⅵa基因进行检测.基因测序确定其基因型,探讨耐药基因与耐药表型的关系.结果 259株KPN中213株对氨基糖苷类抗生素耐药,其中对庆大霉素、妥布霉素、奈替米星和阿米卡星的耐药率分别为83.1%,68.5%,56.3%和44.1%.213耐药菌株中12株携带AAC(3)-Ⅰ基因,阳性率为5.6%;28株携带AAC(3')Ⅵa基因,阳性率为13.1%;103株携带AAC(6')-Ⅰb基因,阳性率48.4%;30株携带AAC(3")-Ⅰ基因,阳性率为14.1%;5株同时携带AAC(3)-Ⅰ和AAC(6')-Ⅰb基因.结论 湖南邵阳地区临床分离肺炎克雷伯菌氨基糖苷类抗生素耐药率高,其耐药基因以AAC(6')-Ⅰb基因为主.
Objective Investigate and analyze drug sensitivity of 2012~2013 clinical isolates of KPN in Shaoyang area in Hunan to investigate the aminoglycoside acetyltransferase gene (AAC) in the aminoglycoside drug resistance strains.Methods 259 KPN clinical isolates were collected from the People's Hospital of Xinshao County of Shaoyang area,People's Hospital of Dongkou County and the Attached Hospital of Shaoyang Medical College during August in 2012 to December in 2013,detecting the drug sensitivity by drug sensitivity and K B disk diffusion method.Genes of AAC (3)-Ⅰ,AAC (6')-Ⅰb,AAC (3')-Ⅰ,AAC (3') Ⅵa were detected by PCR methods and Gene sequence.Results In of 259 strains 213 were aminoglycoside antibiotic resistant strains,the resistance to gentamicin,tobramycin,netilmicin,amikacin rate were 83.1%,68.5 %,56.3%,44.1% respectively.12 of the 213 strains (5.6%) carried AAC (3) Ⅰ gene;28 (13.1%) strains carrying AAC (3')-ⅥⅥ a gene;103(48.4%) strains carrying AAC (6')-Ⅰb gene,and 30 (14.1%) strains carrying AAC (3")-Ⅰ gene.5 strains carry AAC (3)-Ⅰ and AAC (6')-Ⅰb gene.Conclusion According to studies in Shaoyang,Hunan province,the resistance rates of clinical isolates Klebsiella Pneumoniae to Aminoglycoside antibiotics is very high because of the main resistance gene AAC(6')-Ⅰb.
