摘要
目的 探讨检测细胞因子干扰素(interferon,IFN)γ和白细胞介素(Interleukin IL)4的表达在急性胰腺炎(AP)的病情严重程度及疗效评估中的价值.方法 2009年11月~2013年6月,采用酶联免疫吸附试验(ELISA)分别测定32例轻型胰腺炎(MAP组)与18例重症胰腺炎(SAP组)患者住院第1,4,7天和15例正常健康人(正常对照组)的IFN γ和IL 4表达水平.结果 MAP组中住院第1,4,7天IFN γ的检测结果(ng/L)分别为31.47±4.35,36.92±4.12,49.09±4.97,IL 4的表达水平(ng/L)为54.72±5.52,45.26±4.89,39.79±3.85.SAP组中住院第1,4,7天IFN γ表达水平(ng/L)为23.97±5.39,26.29±3.47,33.64±4.47;IL-4的表达水平(ng/L)为70.45±5.05,65.99±4.58,56.23±4.23;正常对照组IFN γ和IL 4的检测结果(ng/L)分别为49.26±20.67,35.15±16.32.与正常对照组相比,MAP组(除外第7天)和SAP组患者外周血IFN-γ表达降低,IL 4表达升高,差异有统计学意义(t=0.753~4.288,P<0.05);相同时间点,SAP组各检测值与MAP组比较差异有统计学意义(t=3.248~11.416,P<0.05).结论 AP的病情进展与患者血清IFN-γ水平降低或者IL 4水平增高有关,检测细胞因子IFN-γ和IL 4有助于判断AP的病情严重程度和治疗效果.
Objective To explore the value of detection of Interferon (IFN)-γ and Interleukin(IL)-4 expression in acute pancreatitis(AP) severity and efficacy evaluation.Methods From May 2009 to June 2013,using enzyme-linked immunosorbent assay (ELISA),the expression levels of Th1-type cytokines interferon-γ(IFN-γ) and Th2 cytokines interleukin (Interleukin IL) 4 of 32 cases of mild acute pancreatitis (MAP group)and 18 cases of severe acute pancreatitis (SAP group)at 1,4,7 d hospitalized as well as 15 cases of normal healthy people (control group) were determined.Results In MAP group hospitalization1,4,7d,thetest results ofIFN-γ (ng/L) were 31.47±4.35,36.92±4.12,49.09±4.97,and those results of IL-4 (ng/L) were 54.72±5.52,45.26±4.89,39.79±3.85.In SAP group hospitalization 1,4d and 7d,the test results of IFN-γ (ng/L) were 23.97±5.39,26.29±3.47,33.64±4.47,and those results of IL 4 (ng/L) were 70.45±5.05,65.99±4.58,56.23±4.23.In the normal control group,the test results of IFN-γ and IL 4 (ng/L) were 49.26±20.67 and 35.15±16.32 respectively.Compared with normal control group,the expression of Th1 type cytokines IFN-γ was decreased and that of Th2 type cytokine IL 4 was increased in MAP group (except for the 7th day) and SAP group patients,and the difference was statistically significant (t=0.753~4.288,P<0.05);the test results of each same time were statistically significant differences between SAP group and MAPgroup (t=3.248~11.416,P<0.05).Conclusion The progression of acute pancreatitis was related to the decreasion of IFN γ or increasion of IL-4 and combined detection of IFN-γ and IL 4 is beneficial to judge the extent of serious condition and treatment of acute pancreatitis.
