早期联合干预对妊娠期甲状腺功能减退孕妇妊娠结局的影响研究 被引量:13

Study on the effect of early comprehensive intervention on pregnancy outcome of pregnant women with hypothyroidism

摘要 目的:探讨孕早期联合干预对改善妊娠期甲状腺功能减退孕妇妊娠结局的效果。方法:选择在河北医科大学第四医院门诊建档的妊娠6~12周甲状腺功能减退孕妇160例,随机分为研究组和对照组,每组各80例。对照组孕妇接受常规诊治,包括孕检和药物治疗;研究组增加早期、专人、系统、联合干预,即实施定人、定量、定期、定时进行知识、生理、心理、饮食、运动等全方位干预。观察并比较两组孕妇孕期并发症及低体重儿发生率。结果:研究组孕妇流产、早产、子痫前期、糖耐量异常、妊娠期贫血及低体重儿发生率明显低于对照组,差异有统计学意义(P〈0.05)。结论:对妊娠期甲状腺功能减退孕妇实施早期、专人、系统、联合干预可显著降低甲状腺功能减退孕妇孕期并发症及低体重儿发生率,值得推广应用。 Objective: To explore the effect of early comprehensive intervention on improvement of pregnancy outcome of pregnant women with hypothyroidism. Methods: A total of 160 pregnant women of 6- 12 gestational weeks with hypothyroidism were selected from the hospital and randomly divided into study group and control group,80 women in each group. The women in control group were treated with conventional therapy,including pregnancy examination and drug therapy; while the women in study group were provided with early,personnel,systematical and joint intervention,including fixed,regular,quantitative,regular knowledge,physiological,psychological,dietary,exercise and other comprehensive intervention measures. The pregnancy complications and the incidence rates of low birth weight infants in the two groups were observed and compared. Results: The incidence rates of abortion,premature birth,preeclampsia,impaired glucose tolerance,gestational anemia and low birth weight infants in study group were statistically significantly lower than those in control group( P〈0. 05). Conclusion: Early,personnel,systematical and joint intervention carried out among pregnant women with hypothyroidism can significantly reduce the incidence rates of pregnancy complications and low birth weight infants,which is worth popularizing and applying.
