摘要
目的 检测热休克蛋白70 (HSP70)和葡萄糖调节蛋白78(GRP78)在肝细胞癌(HCC)中的表达并探讨其与临床病理资料及预后的关系.方法 采用Western blot和免疫组织化学染色的方法检测HCC及其癌旁肝组织中HSP70和GRP78的表达,分析其与临床病理特征的关系;应用Kaplan-Meier法及多因素Cox比例风险模型分析HSP70和GRP78表达与预后的关系.结果 HSP70及GRP78蛋白在HCC组中的相对表达量明显高于癌旁肝组织(0.879±0.556比0.240±0.156,P<0.01;0.515±0.353比0.202±0.132,P<0.01);HSP70阳性表达与肿瘤的大小、乙型肝炎病毒(HBV)感染、Edmondson分级、分化及TNM分期密切相关(P<0.05),GRP78阳性表达与Edmondson分级、分化、微血管侵犯、TNM分期及复发密切相关(P<0.05).单因素生存分析显示HSP70和GRP78蛋白阳性表达组生存率明显低于阴性表达组(P<0.05);多因素Cox回归分析结果显示TNM分期、肿瘤分化程度、肿瘤大小及复发是反映肝癌患者预后的独立危险因素,其相对危险度分别为1.502、1.971、2.267及4.746.结论 HSP70和GRP78过表达参与了HCC的发生和发展.
Objective To investigate the clinicopathological significance of the expression of heat shock protein 70 (HSP70) and glucose regulated proteins 78 (GRP78) in surgically resected hepatocellular carcinoma (HCC).Methods The expression of HSP70 and GRP78 were examined by Western blot and immunohistochemistry.The correlation between HSP70 and GRP78 expression and clinicopathological parameters in HCC were analyzed.Survival data were analyzed using the Kaplan-Meier method and multivariate Cox proportional hazards model.Results The relative expression levels of HSP70 and GRP78 protein in HCC tissues were significantly higher than that in peri-cancer liver tissues (0.879 ± 0.556 vs.0.240±0.156,P〈0.01; 0.515 ±0.353 vs.0.202 ±0.132,P〈0.01).The positive expression of HSP70 was correlated with tumor size,Hepatitis B Virus (HBV) infection,Edmondson grade,differential degree and TNM stage in HCC (P 〈 0.05).The positive expression of GRP78 was correlated with Edmondson grade,differential degree,vascular invasion,TNM stage and recurrence (P 〈 0.05).Survival analysis showed that HSP70-positive and GRP78-positive groups had a significantly shorter overall survival time than HSP70-negative and GRP78-negative groups (P 〈 0.05).Multivariate Cox regression analysis showed that the TNM stage,differential degree,tumor size and recurrence were significant prognostic factors for HCC,and the risk ratios were1.502,1.971,2.267 and 4.746,respectively.Conclusion Overexpression of HSP70 and GRP78 may be involved in the development and progression of HCC.
