摘要
目的研究肾功能指标联合检测在高血压患者早期肾损伤中的应用价值。方法选择60例原发性高血压患者为高血压组,60例体检正常者为对照组,比较分析两组尿微量清蛋白(mAlb)、尿酸(UA)、尿N-乙酰-D氨基葡萄糖苷酶(NAG)、血清胱抑素C(Cys C)、尿β2微球蛋白(β2-MG)等指标。结果高血压组患者的mAlb、UA、NAG、Cys C、β2-MG等指标水平高于对照组(P<0.001)。mAlb+UA+NAG+Cys C+β2-MG联合检测高血压早期肾损伤阳性率为68.3%(41例),明显高于mAlb、UA、NAG、Cys C、β2-MG等各单项检测阳性率(P<0.01)。结论mAlb、UA、NAG、Cys C、β2-MG等指标是临床诊断高血压患者早期肾损伤的常见敏感指标,肾功能指标联合检测阳性率更高,方法简单易行,对高血压性肾病的早期诊断具有十分重要的意义,值得进一步推广应用。
Objective To investigate the application value of joint detection of renal function in early renal damage of hy- pertension patients.Methods 60 patients with essential hypertension were selected as hypertension group, and 60 healthy people proved by physical examination were selected as control group. Then, their urinary albumin excretion （ mAlb ）, Uric Acid （ UA）, urinary N-acetyl-B-D-glucosaminidase （ NAG ）, eystatin C （ Cys C ） and urine 132 microglobulin （ 132-MG ） of the two groups were compared.Results Levels of mAlb, UA, NAG, Cys C and 132-MG of the hypertension group were higher than those of the control group（ P〈O. 001 ）. Joint detection of mAlb＋UA＋NAG＋Cys C＋132-MG in early hypertensive renal in- jury showed that the positive rate was 68.3% （41 cases）, significantly higher than that of single detection of mAlb, UA, NAG,Cys C and 132-MG（ P〈0. 01 ）.Conclusion Indexes such as mAlb, UA, NAG, Cys C, 132-MG were sensitive indexes in the clinical diagnosis of early renal injury of hypertensive patients.And joint detection of renal function has higher positive de- tection rate.This method is easy to operate and is of significance in early diagnosis of hypertensive nephropathy.Thus, it is worthy of further promotion and application.
