摘要 目的了解重度烧伤患者的血生化指标改变,探讨烧伤临床诊治有实用价值的监测指标。方法选择2012年1月~2013年11月于杭州整形医院治疗的126例重度烧伤患者作为烧伤组,同期体检健康者100例作为对照组。观察两组呼吸频率、心率、血压。检测两组患者的白蛋白(ALB)、总蛋白(TP)、空腹血糖(GLu)、二氧化碳(CO2)、尿酸(UA)、门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、高密度脂蛋白胆固醇(HDL-C)、血清总胆固醇(CHOL)及血NO浓度,并对两组的体征指标和生化指标进行比较和分析。结果烧伤组患者的呼吸频率、心率、舒张压、收缩压均显著大于对照组,差异有统计学意义(P〈0.05)。烧伤组ALB、TP、CO2、UA、HDL-C、CHOL结果显著低于对照组,差异均有统计学意义(P〈0.05或P〈0.01);烧伤组GLu、AST、ALT及血NO浓度结果显著高于对照组,差异均有统计学意义(P〈0.05或P〈0.01)。结论通过烧伤患者的体征指标和血生化指标监测,可给烧伤临床的诊断治疗、伤情监测及疗效判断提供有实用价值的科学数据,有利于临床采取及时有效措施减少烧伤的危害。 Objective To study the changes of blood biochemical indices of patients with severe burns, and understand the practical values of those biochemical indices in monitoring clinical situations for patients with burns. Methods From January 2012 to November 2013, in Hangzhou Plastic Hospital, 126 patients with severe burns were selected as the burn group, and 100 persons for normal physical examination purpose as the control group. The respiratory rate, heart rate and blood pressure of both groups were observed. Albumin (ALB), total protein (TP), fasting glucose (GLu), carbon dioxide (CO2), plasma NO concentration, uric acid (UA), aspartate amino transferase enzyme (AST), alanine amino transferase enzyme (ALT), high density lipoprotein cholesterol (HDL-C) and serum cholesterol (CHOL) of both groups were detected. Signs and blood biochemical parameters between the two groups were compared and analyzed. Results The respiratory rate, heart rate, diastolic and systolic blood pressure of the burn group were higher than those of the control group, the differences were statistically significant (P 〈 0.05). The values of ALB, TP, CO2, UA, HDL-C and CHOL of the burn group were significantly lower than those of the control group, and GLu, AST, ALT and plasma NO concentration of the burn group were higher than those of the control group, the differences were statistically significant (P 〈 0.05 or P 〈 0.01). Conclusion It is useful to detect signs and blood biochemical indices of burn patients for monitoring the risks and it is good for reducing the burn hazards.
