摘要
目的探讨清肺承气汤对重症腹腔感染大鼠肺损伤保护作用的可能机制。方法 30只Wistar大鼠随机分为假手术组、模型组和中药组,每组10只。模型组和中药组采用人工胃液联合大肠杆菌腹腔内注射制备大鼠腹腔感染模型,假手术组以等量10%BaSO4营养肉汤取代菌液注射。中药组于造模前1天予清肺承气汤溶液(每毫升含生药量1.5g)灌胃,每只大鼠2ml/次,3次/天;于造模前30min再给药1次,总共给药4次。假手术组及模型组给予等量生理盐水。造模后6h检测各组大鼠肺泡灌洗液(BALF)中蛋白含量、细胞学分类、内毒素水平、细胞间黏附分子-1(ICAM-1)、单核细胞趋化蛋白-1(MCP-1)浓度,肺组织湿/干重(W/D)比值,并观察肺组织病理学改变。结果模型组与中药组BALF中蛋白含量、中性粒细胞较假手术组明显升高(P<0.05)。模型组BALF中内毒素、ICAM-1及MCP-1水平、W/D比值较假手术组明显升高(P<0.01);中药组大鼠内毒素、MCP-1水平、W/D比值较模型组明显降低(P<0.01)。中药组肺泡间质水肿、出血、炎细胞浸润及肺泡壁纤维组织增生等病理损伤均较模型组减轻。结论清肺承气汤可减轻重症腹腔感染大鼠所致的急性肺损伤,其可能与降低大鼠BALF中内毒素、MCP-1水平有关。
Objective To discuss the0. possible protective mechanism of Qingfei Chengqi Decoction on acute lung injury in rats with severe abdominal infection. Methods Totally 30 Wistar rats were randomized into the sham operation group, model group and TCM group, with 10 in each. The model group and TCM group were treated with simulated gastric fluid and E. coli intraperitoneal injection to create abdominal infection model. The sham operation group was treated with equal volume of 10% BaSO4 nutrient broth. One day before modeling, TCM group was gavaged with Qingfei Chengqi Decoction solution ( 1.5 g crude drug/ml) 2ml/time for each rat and 3 times daily. The TCM group was given gastric administration again 30min before modeling together 4 times. The sham operation group and model group were given equal volume of normal saline. Six hours after modeling, the protein level, cytological classi- fication, endotoxin level, intercellular adhesion molecule-1 ( ICAM-1 ), monocyte chemoattractant protein-1 ( MCP- 1 ) and lung wet/dry weight (W/D) ratio in bronchoalveolar lavage fluid (BALF) were detected in each group. The pathological changes of lung tissue were observed. Results Comparing with the sham operation group, the protein level and neutrophil level in BALF were significantly increased in the model group and TCM group ( P 〈 0. 05 ). Com- paring with the sham operation group, the levels of endotoxin, ICAM-1, MCP-1 and W/D ratio were significantly in- creased in the model group ( P 〈 0.01 ). Comparing with the model group, the levels of endotoxin, MCP-1 and W/D ratio were significantly decreased in TCM group (P 〈 0. 01 ). Comparing with the model group, the interstitial edema, hemorrhage, inflammatory cell infiltration, fibrous hyperplasia of alveolar wall and other pathological lesions were alle- viated in TCM group. Conclusion Qingfei Chengqi Decoction can reduce acute lung injury in rats with severe ab- dominal infection, which may be relevant to decreasing the levels of endotoxin and MCP-1 in BALF.
