摘要
目的通过品管圈活动查找手卫生设施缺陷及不合理布点,运用品管手法降低手卫生设施缺陷率及完善布点。方法 2013年3-5月运用品管手法调查手卫生设施及布点,共调查407处洗手设施硬件配置、温馨提示、洗手流程图等项目及消手液利用率、布点缺陷情况,分析原因并提出整改策略。结果洗手设施配置及消手液布点存在缺陷:407处洗手池包括1 628个调查项目中缺陷总数902项,洗手设施总缺陷率为55.41%,缺温馨提示、缺干手纸、缺洗手液、缺流程图是主要存在的问题;消手液共调查753瓶,在30d内使用完的131瓶,仅占17.40%,布点缺陷调查显示多人间病房门前、床旁、病区门前、急救车、护士站是需改善的重点位置;整改后洗手设施缺陷率由55.41%降低至19.50%;消手液30d内用完的比例由17.40%提高至36.48%,利用率得到提高。结论品管圈活动能有效降低手卫生设施缺陷率;同时消手液合理布点提高了资源的利用率,节约了成本,补充了手卫生设施的不足。
OBJECTIVE To find out the defects of hand hygiene facilities and unreasonable layouts through quality control circle（QCC） so as to reduce the rate of detects of hand hygiene facilities and make the layouts reasonable . METHODS From Mar 2013 to May 2013 ,the hand hygiene facilities and layouts were investigated by using the QCC ,then the hardware configuration of hand hygiene facilities ,warm tips ,hand washing flowcharts ,utilization rate of hand disinfectants ,and defects of layouts were investigated in 407 sites ,the causes were analyzed ,and rec-tification strategies were put forward .RESULTS There were defects in the hand hygiene facilities and hand disin-fectants layout sites ：among the 407 sites of hand washing basins ,the defects existed in 902 of 1 628 survey items with the total defect rate of 55 .41% ,the absence of warm tips ,absence of paper towels ,absence of hand washing solution ,and absence of flowchart were the leading problems .Totally 753 bottles of hand disinfectants were inves-tigated ,131 bottles were used up within 30 days ,accounting for 17 .40% .The survey of defects of layouts indica-ted that the doorways of shared wards ,bedside ,doorways of endemic area ,ambulance ,and nurse station were the key sites that needed to be improved .After the rectification ,the defect rate of hand washing facilities decreased from 55 .41% to 19 .50% ;the 30-day utilization rate of hand disinfectants increased from 17 .40% to 36 .48% . CONCLUSION The QCC can effectively reduce the defect rate of hand hygiene facilities ;the reasonable layouts of hand disinfectants raise the utilization rate of resources ,with the cost saved ,the defects of hand hygiene supple-mented .
