彩色多普勒超声对乳腺良恶性肿块诊断的临床价值 被引量:6

摘要 目的探讨乳腺良恶性肿块的超声声像图特征及其鉴别诊断。方法回顾性分析我院2010年6月~2014年6月收治的乳腺肿块28例的临床资料,并对乳腺肿块的超声声像图特征及血流动力学指标血流搏动指数(PI)、阻力指数(RI)、血流最大速度(Vmax)进行分析。结果 28例乳腺肿块中,经彩色多普勒超声诊断证实良性肿块22例,恶性肿块6例。乳腺良性肿块的超声表现为形态规则,包膜完整,内部回声均匀。乳头、乳晕下实质性小结节呈中高回声,内部不均,边界清晰,邻近大导管可伴有扩张。肿块周边或内部可见彩色血流。恶性肿块边界不清,边缘呈多角形或蟹足样,形态不规则,内部呈低回声,无包膜,肿块呈细点状、斑片状、团状、条状、线状等形态,主要为以细点状多见的钙化灶。CDFI显示肿块周围及内部可见多条动、静脉彩色血流信号,多有动脉穿入肿块内并有弯曲或分支。乳腺良性肿块组患者的PI、RI、Vmax分别明显低于恶性肿块组,差异具有统计学意义(P〈0.05)。乳腺良性肿块组血流分级以0~Ⅰ级为主,恶性肿块组的血流分级以Ⅲ级为主。结论应用彩色多普勒超声诊断乳腺良、恶性肿块诊断准确率高,且操作简便,患者无痛苦,更易接受,值得广泛推广和应用。 Objective To investigate the sonographie characteristics of ultrasonic and differential diagnosis of benign and malignant breast tumors. Methods Retrospectively analyzed the clinical data of 28 cases of breast masses of our hospital from June 2010 to June 2014, and the uhrasonographie features and hemodynamie index pulsatility index (PI), resistance index (RI), peak blood flow velocity (Vmax) were analyzed. Results Twenty-eigtht cases of breast masses, transvaginal color Doppler ultrasound in diagnosis of confirmed benign mass in 22 cases, 6 cases of malignant tumor. Uhrasonography of benign breast masses showed morphological rules, complete capsule, internal echo uniform. Parenchymal nodules nipple and areola, with high echo, internal uneven, clear boundary, the adjacent duct with ex- pansion. Mass around or inside the visible color flow.Malignant tumor boundary was not clear, the edge of a polygon or crab like, irregular shape, internal showed low echo, no capsule, punctate, patchy lesions were small, round shaped, strip, linear form, mainly for the more common in fine punctate calcification. CDFI displayed around the mass and in- ternal visible multiple dynamic, vein color flow signal, had penetrated into the mass within the artery and bending or branch.Benign breast mass in patients with PI, RI, Vmax were significantly lower than the malignant tumor group (P〈 0.05). Benign breast mass flow classification in 0-I grade, malignant tumor group blood flow grade mainly m level. Conclusion The application of color Doppler ultrasonography in diagnosis of breast benign, malignant tumor diagnosis accurate rate is high, and the operation is simple, no pain of patients, more easy to accept, and worthy of extensive promotion and application.
