Nasal intermittent positive pressure ventilation in neonates:present status and future

摘要 新生儿气管插管和机械通气技术的进步,使新生儿呼吸系统疾病如新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)、胎粪吸入综合征(meconium aspiration syndrome,MAS)、新生儿呼吸衰竭的病死率显著降低,然而,气管插管和机械通气的并发症,特别是呼吸机相关性肺炎(ventilator associated pneumonia,VAP)、机械通气导致的肺损伤、支气管肺发育不良(bronchopulmonary dysplasia,BPD)等仍然是导致新生儿预后不良的主要问题。 In recent years,noninvasive respiratory support in neonates to reduce the risk of tracheal intubation and its complications has become a hotspot of medical research. As a new noninvasive respiratory support mode,nasal intermittent positive pressure ventilation( NIPPV) is mainly applied to neonatal apnea,respiratory support after mechanical ventilation and the initial treatment mode of respiratory support. Compared with nasal continuous positive airway pressure( NCPAP),NIPPV further reduces the failure rate of extubation after mechanical ventilation and related complications. However,NIPPV has some deficiencies,such as airway pressure instability,the dangers of gastrointestinal flatulence and intracranial hemorrhage. Further basic and clinical researches are needed for the mechanism,indication and long-term effects of NIPPV.
