摘要
目的探讨4',6-二脒基-2-苯基吲哚(4',6-diamidino-2-phenylindole,DAPI)细胞核染色、Ki-67在甲状腺非典型腺瘤(atypical thyroid adenoma,ATA)中的表达及临床意义。方法采用免疫组化EnVision两步法和免疫荧光染色法分别检测ATA、甲状腺滤泡癌(follicular thyroid carcinoma,FC)中Ki-67增殖指数及DAPI核染色情况。结果 DAPI在低、高核级ATA组织中的阳性率分别为12.50%(2/16)、16.13%(5/31),两组相比差异无统计学意义(χ2=0.01,P>0.05);其在对照组FC中的阳性率为85.71%(6/7),两组相比差异有统计学意义(χ2=13.17,P<0.01);Ki-67在低、高核级ATA组织中的阳性率分别为6.25%(1/16)、6.45%(2/31),二者相比差异无统计学意义(χ2=0.36,P>0.05);而在对照组FC中的阳性率为85.71%(6/7),二者相比差异有统计学意义(χ2=22.19,P<0.01);Ki-67、DAPI表达与ATA患者年龄、结节数量、肿块大小无关。结论联合检测Ki-67和DAPI可能对ATA与FC的鉴别诊断有一定价值。Ki-67、DAPI与ATA核级别、患者年龄、肿块大小和结节数量无关。
Purpose To study the 4', 6-diamidino-2-phenylindole (DAPI) nuclear staining and Ki-67 expression in atypical thyroid adenoma (ATA) and their clinical parameters. Methods Ki-67 proliferative activity and DAPI nuclear staining in ATA and follicular carcinoma (FC) were detected with immunohistochemical EnVision two-step method and immunofluorescence staining respectively. Results DAPI positive rate in ATA with low and high nuclear grade was 12.50% (2/16) and 16.13 % (5/31 ), respectively; there was no significant difference between them ( ~2 = 0. 01, P 〉 0. 05 ). the positive rate of DAPI in FC was 85.71% (6/7), which higher than that in ATA ( ~2 = 13. 17, P 〈 0. 01 ). Ki-67-positive rate in ATA with low and high nuclear grade was 6.25% (1/16) and 6.45% (2/31) with no significant difference between them ( χ2 = 0. 36, P 〉 0.05 ). Ki-67 expression in FC was 85.7 1% (6/7) which was statistically significant as compared to ATA (χ2 = 22. 19, P 〈 0. 01 ). Ki-67 and DAPI in ATA were unrelated to age, nod- ule number, and tumor size. Conclusion The detection of Ki-67 and DAPI expression may be helpful for the differential diagnosis of ATA and FC, which may be not related to nuclear grade, age, tumor size and nodular number.
