Surgical treatment of high-energy closed Pilon fracture

摘要 目的评估手术治疗高能量闭合性Pilon骨折的临床疗效。方法高能量闭合性Pilon骨折患者29例均接受切开复位内固定治疗。受伤至接受手术时间为伤后3h-8d。Ruedi-Allgower分型:Ⅱ型损伤21例,Ⅲ型损伤8例。采用改良Mazur评分评估临床疗效。结果术后改良Mazur评分:优9例,良13例,可6例,差1例;优良率75.9%。29例患者术后均未发生严重切口并发症。结论为降低软组织并发症的风险、提高疗效,手术治疗高能量Pilon骨折需要根据骨折类型和伴发软组织损伤情况选择合适的手术时机和个性化的手术方案。 Objective To evaluate clinical outcomes of high-energy closed pilon fractures treated with surgery.Methods Surgical open reduction and internal fixation were performed in 29 cases with high-energy closed Pilon fracture.The patients were classified as Ruedi-Allgower typeⅡin21cases and type Ⅲ in 8cases,who were operated in 3 hours to 8 days after trauma.Results Evaluated with modified Mazur scoring system,the excellent results were seen in 9cases,good in 13 cases,fair in 6cases,and poor in one case.The overall excellent-good rate was 75.8%.Conclusion To minimize the risk of soft-tissue complications and get better clinical outcomes in treating high-energy pilon fractures,properly timing of surgery and individualized operative plan should be selected on the basis of the type of fractures and severity of concomitant soft-tissue injury.
