摘要
目的:观察用滋肾清肝明目经验方治疗老年性黄斑变性的临床效果。方法:选取2011年4月—2014年2月就诊于我院的经中医临床辨证确诊为肝肾亏损型老年性黄斑变性患者86例108只眼,随机分为对照组和观察组患者各43例54只眼。对照组患者给予抗氧化剂,如维生素C、维生素E治疗,并补充锌剂;观察组患者在对照组的基础上给予以滋肾清肝明目经验中药汤剂治疗,1剂/d,15 d为1个疗程,治疗4个疗程。治疗后观察两组患者治疗前后视物模糊、双目干涩、头晕目眩、腰膝酸软、肢体麻木、耳鸣或耳聋等主要临床症状的改善情况及证候积分的变化情况,比较两组患者治疗前后视力的分布情况及临床治疗的有效率。结果:两组患者治疗后视物模糊、双目干涩、头晕目眩、腰膝酸软、肢体麻木、耳鸣或耳聋等主要临床症状均较治疗前有所改善,各证候积分均有所下降,观察组的改善程度更为明显,差异有统计学意义(P<0.05);经治疗后,两组患者的视力分布均有所改善,以观察组的改善程度更为明显,差异有统计学意义(P<0.05);观察组有效率为90.7%,对照组的有效率为68.5%,观察组的有效率明显高于对照组,两组有效率比较,差异有统计学意义(P<0.05)。结论:本研究所用滋肾清肝明目经验方功可滋肾填精、清肝明目,其临床应用能有效缓解患者症状,提高患者的视力水平,效果满意,体现出中医药辨证论治的优势与特色,效果满意,值得临床推广应用。
Objective：To investigate the clinical efficacy of treating patients of age-related macular degeneration with the tonifying kidney and regulating liver prescription.Methods：86 liver and kidney deficiency type of age-related macular degeneration cases（108 eyes） were randomly divided into a control group of 43 cases and a observation group of 43 cases from April 2011 to February 2014.Patients in the control group were treated with antioxidant and zinc for treatment,such as Vitamin CE,zinc gluconate tablets and so on while patients in the observation group were treated with self-made decoction,tonifying kidney and regulating liver prescription for treatment on the basis of foundation treatment,1 agent/day,15 days for a course,four courses for treatment.We observed the improvement of clinical symptoms,mainly including blurred vision,dry eyes,dizziness,lumbar debility,numbness,tinnitus and deafness and so on.We compared the change of symptom score in pretherapy and post-treatment,observed the distribution of vision and calculated the effective rate of the two groups.Results：The main clinical symptoms of the patients in two groups,such as blurred vision,dry eyes,dizziness,lumbar debility,numbness,tinnitus and deafness were improved clearly,and the symptom scores went down clearly.The degree of change in the observation group was more pronounced.The difference was significant（P0.05）.The distribution of vision in two groups after treatment was improved.The degree of change in the observation group was more pronounced and the difference is significant（P0.05）.The efficiency for patients in the control group was 68.5% while for patients in the observation group was 90.7%.The clinical efficacy of the observation group was higher than that of the control group and the difference was significant（P0.05）.Conclusion：The tonifying kidney and regulating liver prescription has the function of tonifying kidney,replenishing essence,clearing liver and improving vision.It can improve the clinical symptoms,raise the level of the patients＇ vision and has the satisfactory clinical effect,which shows the advantages and characteristics of TCM.It is worthy of further research and development.
