寰枢椎椎弓根螺钉棒内固定技术治疗寰枢椎失稳

摘要 目的探讨经寰枢椎椎弓根螺钉棒内固定技术治疗寰枢椎失稳的可操作性和临床疗效。方法对20例寰枢椎不稳的患者应用寰枢椎椎弓根螺钉内固定技术治疗,术前均行X线、CT、MRI等影像学检查;术中在直视下行寰枢椎椎弓根置钉,复位固定,椎板后弓植骨;术后颈托固定3个月。结果全组病例未发生椎动脉,脊髓损伤,术后临床症状得到不同程度的改善,随访6~34个月随访,平均20.4个月,患者均在3~6个月时寰枢椎骨性融合,未发现螺钉松动、断钉和寰枢椎再移位现象。结论经寰枢椎椎弓根螺钉内固定技术是治疗寰枢椎失稳的有效方法。 Objective To explore the surgical feasibility and clinical outcome ofatlantoaxial pedicle screw instrumen- tation for the treatment of atlantoaxial instability. Methods 20 cases with atlantoaxial instability were treated surgically with attantoaxialpedicle screw instrumentation were reviewed. Preoprative CT and radiograph as well as intraoperative screw placement and bone graft were administered in all cases. Cerv - ical collars as external support were used 3 months. Results All patients were follow - ed up form 6 to 34 months, averaged 20. 4 months. There were no neural or vascularinjury was observed. The clinical symptom was improved in some extent. No cases of implant failure were ob- served, and all patients had a solid bone fusion on X - ray taken 3 to 6months after surgery without the complications of internal fixation failt - ure and redislocation of atlas. Conclusions The C1 - C2 pedicle screw fixation has theadvantages of direct screw placement, short - segment fixation, intraoperative reduction and high fusion rate, which could be used as an effective method for atlantoaxial ins -tability.
