2010—2013年潍坊市医院透析用水与透析液微生物污染状况调查 被引量:3

Investigation on microbiological pollution of dialysis water and fluid in hospitals of Weifang city in 2010-2013

摘要 目的了解2010—2013年潍坊市医院透析用水和透析液微生物污染状况,为保障血液透析安全和医院感染控制提供理论依据。方法以透析机反渗水输水管末端透析用水与透析器出口透析液为样本,对潍坊市2010年15家、2011年20家、2012年22家、2013年24家医疗机构展开每年一次的定期监测。对样本采用鲎试剂检测法测定内毒素含量,采用营养琼脂倾注平板法测定菌落总数。计数资料采用χ2检验,P<0.05为差异有统计学意义。结果 2010—2013年共检测透析用水81份,总合格率为91.4%,逐年合格率分别为100.0%、95.0%、77.3%、95.8%。其中2012年合格率最低,各年比较差异有统计学意义(P<0.05)。共检测透析液81份,总合格率为93.8%,逐年合格率分别为100.0%、95.0%、81.8%、100.0%。其中2012年合格率最低,各年比较差异有统计学意义(P<0.05)。2010—2013共检测全市5家市级医院、19家县级医院透析用水和透析液162份,其中市级医院合格率为100.0%,县级医院总合格率为88.7%,其差异有统计学意义(χ2=6.847,P<0.05)。结论 2010—2013年潍坊市部分医院透析用水和透析液微生物指标存在不合格现象,县级医院问题突出,建议相关部门加强监督管理,保障血液透析安全。 Objective To investigate the microbiological pollution of dialysis water and fluid in hospitals of Weifang city during 2010 -2013, and to provide a theoretical basis for promoting dialysis safety and infection control. Methods Sampies were dialysis water collected from the RO ( Reverse Osmosis) system outlet and dialysis fluid collected from the terminal outlet of dialysis machines. Regular monitoring was carried out annually in medical institutions ( 15 hospitals in 2010, 20 hospitals in 2011, 22 hospitals in 2012 and 24 hospitals in 2013). Endotoxin was tested by TAL (Tachypleus Amebocyte Lysate) while total bacteria were counted on nutrient agar plate.x^2 test was used for count data and the result of P 〈0. 05 was considered significantly different. Results A total of 81 samples of dialysis water were tested during 2010 to 2013, the overall qualification rate was 91.4% ; and the qualification rate was 100.0% ,95.0% ,77.3% ,95.8% year by year, which was the lowest in 2012 and showed significant difference between years( P 〈0.05). A total of 81 samples of dialysis fluid were tested during 2010 to 2013 ,the overall qualification rate was 93.8% ;and the qualification rate was 100.0% ,95. 0% ,8l. 8% ,100.0% year by year,which was the lowest in 2012 and showed significant difference between years(P 〈0. 05 ). A total of 162 samples of dialysis water and fluid were examined from 5 municipal hospitals and 19 county hospitals during 2010 - 2013. The qualification rate was 100% in municipal hospitals and 88.7% in county hospitals. There was al- so significant difference between the municipal hospitals and municipal hospitals (x^2 = 6. 847, P 〈 0.05 ). Conclusions Some of the dialysis water and dialysis fluid are found to be unqualified in hospitals of Weifang, especially in county hospitals. Therefore, supervision should be strengthened by the authorities to protect the safety of hemodialysis .
