生物反馈联合电刺激治疗对全子宫切除术后盆底功能康复的疗效 被引量:13

Efficacy of Biofeedback Combined with Electrical Stimulation Therapy for the Rehabilitation of Pelvic Floor Function after Total Abdominal Hysterectomy

摘要 【目的】探讨生物反馈联合电刺激治疗对经腹全子宫切除术后患者盆底功能康复的疗效。【方法】采用前瞻性病例对照研究方法,将2011年6月至2012年12月在湖南省妇幼保健院因子宫良性疾病行经腹全子宫切术的135例患者随机分为研究组(65例)和对照组(70例)。研究组于术后3个月开始进行生物反馈联合电刺激治疗2个疗程,对照组进行一般健康宣教。分别于术前、术后3个月、术后6个月、术后12个月询问两组患者压力性尿失禁(SUI)发生情况、检测盆底肌肉电生理指标。【结果】两组术后6个月、12个月比较,对照组尿失禁发生率较研究组均明显偏高,且两组比较差异有显著性( P <0.05);研究组盆底肌肉肌力术后6个月及12个月均高于对照组,且两组比较差异有显著性( P <0.05),研究组术后3个月肌电位值低于术前,且差异有显著性( P <0.05),而术后6个月及12个月均明显高于术前,且差异有显著性( P <0.05)。【结论】生物反馈联合电刺激治疗可以降低子宫切除术后SUI发生率,提高盆底肌肉肌力,是预防及治疗子宫全切术后盆底功能障碍性疾病(PFD )安全、有效的方法。 [Objective]To explore the efficacy of biofeedback combined with electrical stimulation therapy for the rehabilitation of pelvic floor function after total abdominal hysterectomy .[Methods] Totally 135 pa‐tients with benign uterine disease undergoing total abdominal hysterectomy surgery in Hunan maternal and child health hospital from June 2011 to Dec .2012 were randomly divided into study group( n=65) and control group( n=70) by using a prospective case‐control study method .The study group received biofeedback com‐bined with electrical stimulation therapy for 2 cycles at 3 months after operation ,while the control group was given general health education .The occurrence of stress urinary incontinence (SUI) was inquired and electro‐physiological indexes of pelvic floor muscle was measured in two groups before ,3 months ,6 months and 12 months after operation .[Results]Compared with the study group ,the occurrence of SUI in control group at 6 months and 12 months after operation were increased obviously ,ant there were significant differences between two groups( P〈0 .05) .Pelvic floor muscle strength in study group at 6 months and 12 months after operation were higher than those in control group ,and there were significant differences between two groups ( P 〈0 .05) .Muscle potential value in study group at 3 months after operation was lower than that before operation, and there was significant difference( P 〈 0 .05) .Muscle potential value in study group at 6 months and 2 months after operation was obviously higher than that before operation ,and there was significant difference( P〈0 .05) .[Conclusion] Biofeedback combined with electrical stimulation therapy can reduce the incidence of SUI and improve pelvic floor muscle strength ,so it is a safe and effective method for preventing and treating pelvic floor dysfunction(PFD after total hysterectomy .
