骨与软组织急性感染脉冲冲洗对创面病原菌的清除作用 被引量:6

Scavenging of wound pathogens by pulse lavage for acute infection of bone and soft tissues

摘要 目的分析并探讨脉冲冲洗术清除骨与软骨组织急性感染创面致病菌的清除效果,从而更好的清除患者感染创面病原菌。方法选取2008年5月-2014年5月医院急性骨与软组织感染患者70例;首先进行常规清除之后,采用脉冲冲洗术进行冲洗,将术前、常规清创后、脉冲冲洗后的渗出液作为标本进行病原菌检测,数据采用SPSS 19.0软件进行分析。结果术前共检出病原菌70株,其中革兰阳性球菌37株、革兰阴性杆菌33株;常规清除术后共检出病原菌34株,革兰阳性球菌11株、革兰阴性杆菌23株,与术前相比病原菌显著下降(P<0.05),脉冲冲洗后共检测出病原菌6株,革兰阳性球菌及革兰阴性杆菌各3株,两种菌均有显著下降(P<0.05),经脉冲冲洗后清除率达91.43%。结论脉冲冲洗术清除急性骨与软骨组织感染创面致病菌清除效果显著,且对革兰阳性球菌与革兰阴性杆菌均有较强清除能力,临床上值得推广。 OBJECTIVE To analyze and discuss the scavenging effect of pulse lavage on wound pathogens for acute infection of bone and cartilage tissues ,in order to better clean wound pathogens in infected patients .METHODS Totally 70 patients with acute bone and soft tissue infections admitted in our hospital during May 2008-May 2014 were enrolled .First ,the wounds were routinely cleaned ,then were washed by pulse lavage .Exudates before sur‐gery ,after conventional clearance and after pulse lavage were taken as samples for pathogen detection .Data were analyzed with the software SPSS 19 .0 .RESULTS Preoperatively ,70 pathogens were detected ,including 37 gram‐positive cocci ,and 33 gram‐negative bacilli .After conventional clearance ,34 pathogens were detected ,including 11 gram‐positive cocci ,and 23 gram‐negative bacilli ,which were decreased significantly compared with preopera‐tive pathogens (P〈0 .05) .After pulse lavage ,6 pathogens were detected ,including 3 gram‐positive cocci ,and 3 gram‐negative bacilli ,these two strains were significantly decreased (P〈0 .05) .The clearance rate was up to 91 .43% after pulse lavage .CONCLUSION The clean effect of pulse lavage on wound pathogens for acute infection in bone and cartilage tissues is remarkable ,which has a strong ability to clean gram‐positive cocci and gram‐nega‐tive bacilli and is worthy for clinical promotion .
