应用等速肌力作为髋关节置换术前功能评估的新指标 被引量:1

摘要 目的研究髋骨关节炎患者髋周肌力与传统影像学分级和功能评分的关系,初步探讨等速肌力作为一种新的关节置换术前功能评估指标的应用。方法选取单侧髋关节骨关节炎患者20例,其中Kellgren和Lawrecne骨关节炎放射线分级Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期各5例,分别进行健侧和患侧的等速肌力测定,并进行Harris评分、VAS评分,将等速肌力指标与临床、影像学资料进行比较。结果 4组患者的峰力矩体重比、力矩加速能、平均功率、总功、患健侧峰力矩比值均符合影像学分级特点(P<0.05)。4组患者的峰力矩体重比与Harris评分、VAS评分具有相关性(0.3≤|r|<0.8)。结论髋骨关节炎患者术前患侧肌肉功能明显低于健侧,且下降程度符合影像学分级。从功能评估角度,等速肌力指标与临床资料反映的疼痛及功能损害严重程度具有相关性。 Objective To observe the association between the tissue balance and X - ray photo of different degrees of patients with hip osteoarthritis, in order to preliminarily study isokinetic muscle strength as a new evaluation index before arthroplasty. Methods Selecting 20 patients with osteoarthritis of hip in only one side into four groups by Kellgren and Lawrecne's Osteoarthritis grading standards. The isoki- netic muscle strength of 20 patients' two sides hip were tested and analyzed, and then Harris Score and Visual Analogue Score were used to compare isokinetic muscle strength index and clinical and X - ray data. Results PT/BW, TAE, AP, TW, the ipsilateral and contralateral peak torque ratio of four groups was in line with the characteristics of image classification ( P 〈 0.05 ). There was correlation of four groups with PT/BW and Harris score and VAS score (0.3 ≤|r| 〈 0.8). Conclusion The muscle strength of osteoarthritis side was significantly lower than that of healthy side. From the angle of function evaluation, there is a correlation between severity of muscle strength index and the pain the clinic and X -ray reflect.
