摘要 目的研究聚合酶链反应在检测阴道细菌中的临床应用。方法患有细菌性阴道炎的妇女,为感染组(n=80);对照组为在我院进行健康体检的妇女(n=81),分别用实时荧光定量PCR方法与革兰染色方法检测阴道分泌物中的加德纳菌,比较两种方法加德纳菌的检出情况。结果对80例感染组和81例对照组采样革兰染色法检测,阳性率分别为73.75%和7.4%;用PCR荧光定量法对2组进行检测,阳性率分别为93.75%和8.64%。PCR荧光定量检测与革兰染色法检测研究组与对照组研究对象加德纳菌阳性率,2种方法检测结果差异均有统计学意义(χ2=115.7,P<0.001;χ2=73.6,P<0.001)。在细菌性阴道炎患者中检测加德纳菌,将荧光定量PCR和革兰染色法检测结果进行比较,荧光定量PCR检测阳性率显著高于培养法(χ2=11.76,P<0.001)。结论实时荧光定量PCR方法检测加德纳菌的准确性高、方法简单易操作,可满足临床诊断细菌性阴道炎的需要,值得在临床上推广应用。 Objective To study the clinical application of the polymerase chain reaction in detection of vaginal bacteria. Meth-ods Women with bacterial vaginosis were taken as the infected group (n=80), and the women who received health checkup at our hospital were taken as a control group ( n=81 ) , gardnerella in vaginal secretions was respectively detected by the method of fluores-cence quantitative polymerase chain reaction ( PCR) as well as Gram staining method, and then the detection results of gardnerella ob-tained by the two methods were compared. Results 80 cases from the infected group and 81 from the control group were sampled and detected by Gram staining, and the result showed that the positive rates were 73. 75% and 7. 4 %, respectively, the two groups were detected by PCR, and the result showed that the positive rates were 93. 75% and 8. 64% respectively. Gardnerella positive rates of the subjects of the infected group and the control group were detected by PCR fluorescence quantitative detection and Gram staining, the differences in detection results of these two methods were statistically significant (χ2=115. 7, P〈0. 001, χ2=73. 6, P〈0. 001). When the gardnerella of patients with bacterial vaginosis was detected, the comparison between the test results of PCR and Gram stai-ning showed that the positive rate of PCR detection was significantly higher than that of culture method (χ2=11. 76, P〈0. 001). Con-clusion Real-time PCR method for detecting Gardnerella proves to be highly accurate, easy to operate and capable of meeting the needs of clinical diagnosis of bacterial vaginosis, and thus, it is worthy of being popularized and applied clinically.
