摘要
目的建立荧光定量PCR 法对呼吸道病原体的快速检测方法,为临床呼吸道疾病的诊断和治疗提供参考.方法:本研究选择2012 年6 月至2013 年6 月到我院呼吸科接受临床检查的102 例患者,收集其咽拭子,提取DNA 或者RNA,根据生物信息学分析结果结合相关文献报道设计出8 组荧光定量PCR 引物及其探针,并对其进行多重荧光定量PCR,快速检测其感染的呼吸道病原体.结果:根据ABI 7300 Real-Time PCR System 给出的结果判断,共检出72 例患者检测出病原体,阳性率为80.39%,其中呼吸道合胞病毒(RSV) 26 例,占31.71%,腺病毒(ADV)17 例,占20.73%,流感病毒(Ful)16 例,占19.51,SARS 冠状病毒(SARS)和H5N1甲型流感病毒(H5N1)检测0 例,肺炎支原体(MP)12 例,占14.63,肺炎衣原体(Cpn)9 例,占10.98%,麻疹病毒(MV)2 例,占2.43%.研究发现8 例(占11.11%)阳性病例为混合感染,2 例同时感染3 种病原体,6 例同时感染2 种病原体.结论:荧光定量PCR 具有快速、特异性强等优点,在呼吸道病原体检测方面有巨大应用价值,值得临床推广.
Objective to develop a rapid detection method of fluorescent quantitative PCR method for respiratory pathogens, and provide reference for clinical diagnosis andtreatment of respiratory diseases. Methods: This study selected 2012 June to 2013 June to our hospital department of respiration, 102 cases underwent clinical examination of patients,collected the throat swabs, extraction of DNA or RNA, according to the bioinformatics analysis results with related literature reports designed 8 sets of fluorescence quantitative PCRprimer and probe, and multiple fluorescence quantitative PCR on it, quick detection of respiratory pathogens of infection. Results: the ABI 7300 Real-Time PCR System judgmentaccording to the results, there were 72 patients detected pathogens, the positive rate was 80.39%, including respiratory syncytial virus (RSV) in 26 cases, accounting for 31.71%,adenovirus (ADV) in 17 cases, accounting for 20.73%, influenza virus (Ful) in 16 cases, accounting for 19.51, SARS coronavirus (SARS) and H5N1 influenza A virus (H5N1) weredetected in 0 cases, pneumonia mycoplasma (MP) 12 cases, accounting for 14.63, Chlamydia pneumoniae (Cpn) in 9 cases, accounting for 10.98%, the measles virus (MV) in 2 cases,accounting for 2.43%. The study found that the 8 cases (11.11%) positive cases of mixed infection, 2 cases and 3 pathogens, 6 cases and 2 pathogens. Conclusion: fluorescencequantitative PCR has the advantages of high speed, strong specificity, has great application value in respiratory tract pathogen detection, is worth the clinical promotion.
