P16蛋白和TCT及HPV基因分型联合检测在宫颈病变筛查中的应用 被引量:9

摘要 目的探讨P16蛋白、液基薄层细胞学检测(TCT)和人类乳头状瘤病毒(HPV)基因分型联合检测在宫颈病变筛查中的应用价值。方法选取2 632例宫颈疾病患者进行TCT检查,采用免疫组化Elivision法检测P16蛋白的表达,用核酸分子快速导流杂交技术检测HPV基因分型。结果 HPV感染总阳性率为26.94%,21种基因亚型均被检出;阳性率居前5位的基因亚型为HPV16型(13.16%)、HPV52型(12.84%)、HPVcp8304型(11.00%)、HPV58型(10.46%)和HPV18型(8.31%),基因亚型以高危型为主,占74.76%;单一和二重感染分别占67.98%和24.26%;细胞学分类诊断为正常+炎症、ASC US、LSIL和HSIL的对象中,HPV感染的阳性率分别为20.98%、38.99%、61.33%和75.51%;正常+炎症、ASCUS、LSIL和HSIL中P16蛋白表达率分别为0.00%、22.02%、44.44%和79.59%;不同宫颈细胞学诊断级别间HPV感染率及P16蛋白表达率差异均有统计学意义(P<0.01)。结论宫颈疾患人群中,HPV感染率较高,且以高危型、单一和二重感染居多;随着宫颈病变程度加重,HPV的感染率和P16蛋白表达率逐渐升高。
