摘要
目的探讨食管鳞状细胞癌(ESCC)患者血浆miR-200c和miR-155的表达水平及二者的临床意义。方法选取2012年9月—2013年5月在郑州大学第一附属医院确诊为ESCC的患者33例(ESCC组)和同期在本院行体检健康者30例(对照组)。采用实时定量反转录聚合酶链反应(qRT-PCR)测定两组受检者血浆miR-200c、miR-155水平,分析二者诊断ESCC的价值,并探讨二者与ESCC患者各临床病理指标间的关系。结果 (1)ESCC组患者血浆miR-200c、miR-155水平均高于对照组〔(13.87±2.27)与(2.43±3.04),(8.09±1.97)与(1.77±2.74);P<0.05〕。(2)受试者工作特征(ROC)曲线分析显示,血浆miR-200c和miR-155诊断ESCC的ROC曲线下面积(AUC)分别为0.853〔95%CI(0.774,0.931),灵敏度为79%,特异度为88%〕和0.771〔95%CI(0.674,0.867),灵敏度为69%,特异度为81%〕。(3)ESCC患者血浆miR-200c水平与病理分期及是否转移有关(P<0.05);而与性别、年龄、分化程度、是否手术无关(P>0.05)。ESCC患者血浆miR-155水平与各临床病理指标均无关(P>0.05)。结论 ESCC患者血浆miR-200c、miR-155较健康者表达水平升高,可作为诊断ESCC潜在的生物标志物;血浆miR-200c水平与ESCC病理进程相关。
Objective To investigate the expression level and clinical significance of plasma miR- 200 c and miR- 155 in patients with esophageal squamous cell carcinoma( ESCC). Methods 33 ESCC patients confirmed in the First Affitiated Hospital of Zhengzhou University( ESCC group) and 30 healthy people underwent physical examination( control group) from September 2012 to May 2013 were selected. Quantitative real- time polymerase chain reaction( qRT- PCR) was used to detect the plasma miR- 200 c and miR- 155 levels,and the value of the two in diagnosing ESCC was analyzed,as well as the relationship between the two indicators and clinical indexes of ESCC patients. Results The expression levels of plasma miR- 200 c and miR- 155 in ESCC group were significantly higher than that in control group 〔( 13. 87 ± 2. 27) vs.( 2. 43 ± 3. 04);( 8. 09 ±1.97) vs.( 1. 77 ±2.74); P 〈0. 05〕. The ability of plasma miR-200 c and miR-155 to discriminate the ESCC from those controls were evaluated by the ROC curve with an AUC of 0. 853 〔95% CI( 0. 774,0. 931),sensitivity 79%,specificity 88% 〕and 0. 771 〔95% CI( 0. 674,0. 867),sensitivity 69%,specificity 81% 〕respectively. Plasma miR- 200 c levels were not associated with gender,age,differentiation degree and operation( P 〉0. 05),but were related to pathological stage and metastasis( P 〈0. 05); plasma miR- 155 levels were not associated with any of the clinical pathological indicators collected( P 〉0. 05). Conclusion The expression levels of plasma miR- 200 c and miR- 155 are significantly increased in patients with ESCC,and they could serve as potential diagnostic biomarkers for ESCC. The plasma miR- 200 c level is associated with the pathological process of ESCC.
